本研究の目的は，障がい者福祉施設の利用者に対して，慢性疾患（高血圧症，糖尿病，動脈硬化症）のリスク層別化を行い，高リスク群に対して疾病管理プログラムを実施し，有効性を検証することである。

The purpose of this study is to stratify the risk of chronic diseases (hypertension, diabetes, arteriosclerosis) for persons with disabilities in welfare facilities, and implement a chronic disease management program for high-risk groups. It is to verify the effectiveness.

対象は20～75歳の男女とし，障がい者福祉施設に通う利用者（高次脳機能障害，知的障害，精神障害），研究参加に同意が得られた方とする．

Subjects are men and women between the ages of 20 and 75, and those with disabilities (neurocognitive disorders, intellectual disability, mental disorders) attending welfare facilities for persons with disabilities and those who have consented to participate in the study

除外基準/Key exclusion criteria

以下のいずれかに該当する場合は除外する。
・意思疎通が不可能な者
・障害や精神疾患により質問に回答できない者
・代諾者（家族または代理人）より同意が得られなかった者

Those who cannot communicate
Those who cannot answer questions due to disabilities or mental illness
Person who did not get consent from the legal representative (family or agent)
