脊髄性筋萎縮症，筋ジストロフィーなどの神経筋疾患患者で，手指でタップ動作あるいはスワイプ動作を行う姿勢をとることが出来る患者

・臨床情報
身長、体重，
運動機能（歩行可能，車椅子使用，臥床状態など），
呼吸状態（自発呼吸，非侵襲的人工呼吸，侵襲的人工呼吸など），
栄養管理状態（経口摂取，経管栄養，胃瘻など），
ほか合併症（心不全など），投薬治療内容

・リハビリテーションで日常臨床の一環として行っている運動機能評価項目：
徒手筋力など，（脊髄性筋萎縮症では以下を評価：拡大版ハンマースミス運動機能評価，改訂版上肢機能評価，握力など）

・臨床検査
血液学的検査，血液生化学

・胸部単純X線，脊柱単純X線、心エコー，心電図，呼吸機能検査（VC,%VCなど）

Respiratory function (voluntary, non-invasive ventilation, invasive ventilation, etc)
Nutritional method (per oral, tube feeding, gastrostomy tube, etc)
Other complication (heart failure, etc), medication

Evaluation of rehabilitation:
Manual muscle test(MMT), Hammersmith Functional Motor Scale-Expanded(HFMSE)(for SMA), revised upper limb mudule(RULM)(for SMA), etc

Clinical laboratory test:
Blood analysis, blood chemistry,

Chest x-ray, spine x-ray, echocardiography, electrocardiography, respiratory function (VC, %VC, etc)
