全身性硬化症において、持続的温熱によって手指末梢の毛細血管密度あるいは形態的変化の有無をキャピラロスコピーを用いて確認する。

選択基準/Key inclusion criteria

日本語
１）20歳以上、且つ90歳未満。
２）2013年アメリカリウマチ学会の全身性強皮症分類基準に合致するレイノー現象を示す患者。
３）大阪大学医学部附属病院外来通院中の患者。
４）本研究の参加にあたり十分な理解の上、本人の自由意思により文書同意された患者。

1. Patients over 20 years old and under 90 years old.
2. Patients who have Raynaud's phenomenon(RP) and meet the 2013 Classification Criteria for Systemic Sclerosis (ACR/EAULAR).
3. Patients visiting the outpatient of Osaka University Hospital.
4. Patients who give written informed consent.

除外基準/Key exclusion criteria

日本語
１）使い捨てカイロを装着する部位に全身性強皮症以外の皮膚異常がある患者。
２）発熱を伴う慢性疾患、感染症を合併する患者。
３）糖尿病を有する患者。
４）自分の意志でデバイスの脱着が出来ない患者。
５）末梢神経障害を有する者、又は過去に末梢神経障害の診断を受けた者。
６）血液透析のため上肢にシャント血管を有する患者。
７）研究責任医師が不適当と判断する患者。

1. Another skin disease except systemic sclerosis at the site of wearing
2. Chronic febrile disease including infectious disease
3. Diabetes mellitus
4. Incapable of taking off the warmer by themselves
5. Peripheral neuropathy of upper extremities or past medical history of that
6. Vascular shunt on the arm for hemodialysis
7. Others (judged as ineligible by the responsible researcher)

主な結果/Results

日本語
指尖部血液中のangiopoietin-1濃度は加温後増加した。この増加は加温終了後4週間目においても持続していた。
キャピラロしコピーによる最前列の毛細血管密度の変動には一定の傾向がなかった。
キャピラロスコピーによる最前列の毛細血管の内、正常なヘアピン形態の血管の割合は加温開始後増加した。

Angiopoietin-1 levels in blood at the fingertip increased after heating. This increase persisted at four weeks after the end of heating.
Capillary density changes in the anteriormost capillaries did not show a consistent trend.
The percentage of capillaries in the anterior-most row with normal hairpin shape increased after the heating.

参加者背景/Baseline Characteristics

1日も介入が入らなかった1例を除いた19名の平均年齢は55.9歳、男性3名、女性16名であった。限局皮膚硬化型SScが14名、びまん性皮膚硬化型SScが5名であった。平均罹病期間は13.3年であった。平均ｍRSSは4点であった。

In nineteen subjects, excluding one case without any intervension for even one day,the average age was 55.9 years old, 3 males and 16 females. There were 14 patients with lcSSc and 5 patients with dcSSc. The mean disease duration was 13.3 years. The average mRSS was 4 points.

参加者の流れ/Participant flow

日本語
20名の全身性硬化症(SSc)患者が参加した。SSc以外の原因による入院のため1名は1日の介入もなく脱落となった。2名の参加者が本人意思により中止となった。17例の参加者が全ての行程を終了した。

Twenty patients with systemic sclerosis (SSc) participated; one dropped out without a day's intervention due to hospitalization for causes other than SSc; two participants withdrew of their own volition; 17 participants completed the entire procedure.

有害事象/Adverse events

以下の有害事象が観察された。皮膚線条１、網状皮斑１、皮膚発赤１、熱傷３、皮膚掻痒感１。
この内、熱傷2件は治療（外用剤）を実施。他は無治療で回復した。

The following adverse evets were observed; skin striae:1, reticular dermatitis:1, skin redness:1, burns:3, skin itching:1. Of these, 2 burns were treated with topical agents. Others recovered without treatment.
