日本語
OCTを用いて冠状動脈を観察したCAD患者のうち、5年以上の長期間を経て、同一部位にOCT評価を行った患者を抽出し、Baseline OCT時に観察された正常冠状動脈病変について、同一部位のFollow-up OCT時と比較することで、正常血管壁から初期アテローム性動脈硬化病変への進展に関連する因子を検討し、冠状動脈における初期アテローム性動脈硬化の病態に関わる因子を探索することである。

The objective of the present study is to investigate the incidence and related factors associated with atherosclerotic progression from NVW to fibroatheroma using long-term serial OCT follow-up data over 5 years.

選択基準/Key inclusion criteria

日本語
神戸大学医学部附属病院にて、2005年4月1日から2019年6月30日までの期間中に、OCTを用いて初回の冠状動脈の評価を行った患者のうち、5年以上の期間をあけて、再度同一部位に対しOCTを用いて冠状動脈の評価を施行することで、長期間における連続した冠状動脈の変化を評価し得た患者を対象とする。

The patients who underwent OCT imaging for native coronary artery were screened, we enrolled the patients who met the following inclusion and exclusion criteria into the present study. The inclusion criteria included patients 1) who underwent serial OCT imaging for native coronary artery over 5 years; 2) who underwent OCT for early atherosclerotic lesions located >5mm away from the stent edges, defined as diameter stenosis <30% on coronary angiography

除外基準/Key exclusion criteria

日本語
1.　初回OCTを施行した後に、冠状動脈バイパス手術を施行した患者。
2.　対象血管に慢性完全閉塞をきたした患者。
3.　解析可能な画像を得ることができなかった患者。
4.　観察期間中に対象血管に対する冠状動脈血行再建術が施行された患者。

The exclusion criteria included patients 1) who had undergone bypass graft surgery; 2) with chronic total occlusion lesions within the target vessel; 3) with poor OCT images; 4) who had undergone coronary revascularization within the same vessel during the follow-up period.

研究対象患者のBaselineおよびFollow-up OCTの画像を長軸/短軸方向ともに一致させ、1mm毎に解析を行う。BaselineからFollow-upの間にアテローム性動脈硬化が進展した領域（Atheroma progression）と進展しなかった領域を比較し、Atheroma progression と関連するBaseline OCTの画像的特徴を明らかにする。

Baseline OCT images were evaluated every 1mm slice. Then, matched follow-up OCT images were selected. The cross-sections were diagnosed as Progression when NVW in the cross-section reduced by >30 degrees from baseline OCT to >5-year follow-up OCT. We investigated the incidence and related factors associated with atherosclerotic progression from NVW to fibroatheroma using long-term serial OCT follow-up data over 5 years.
