本研究の目的は、Sit-up試験を応用し電動傾斜ベッドを使用せずに脳卒中患者の起立性低血圧を安全に評価できる手法を開発することである。

選択基準/Key inclusion criteria

日本語
1. 初発脳卒中
2. 発症後1ヶ月～6ヶ月。
3. Mini-Mental State Examinationが24点以上で、研究内容を理解できる。
4. 研究参加への同意が得られる。

1. First-ever stroke
2. Stroke within 1 to 6 months
3. No severe cognitive impairment (a Mini-Mental State Examination score of 24 or more)
4. Consent to participate in this study

除外基準/Key exclusion criteria

日本語
1. 関節可動域制限や疼痛などの理由により、10分の背臥位姿勢および3分間の端座位および電動傾斜ベッドでの立位姿勢を取れない。
2. 両片麻痺を呈している。
3. 研究協力困難な精神疾患を有する。
4. コントロール不良の不整脈、高血圧症、糖尿病を有する。
5. 脳卒中以外の中枢神経疾患の既往がある（脊髄損傷、パーキンソン病、脊髄小脳変性症）。
6. 上腕骨骨折や乳がん術後などの理由により、非麻痺側上腕での血圧測定ができない。
7. その他、研究者および主治医が研究被験者として適当でないと判断した者。

2. Bilateral hemiparetic stroke
3. Mental impairment leading to inability to corporate
4. Unstable medical conditions
5. Other neurologic conditions in addition to stroke
6. Unable to measure blood pressure from the nonparetic arm due to a fracture of the humerus and/or after surgery for breast cancer
7. Inappropriate patients due to other reasons

主な結果/Results

日本語
ヘッドアップティルト試験中に、8名が収縮期起立性低血圧を有し、3名が拡張期起立性低血圧を有していた。
ROC解析の結果、Sit-up試験を用いて起立性低血圧の有無を判別するカットポイントは、10 mmHg以上の収縮期血圧低下(AUC ＝ 0.915, 感度 = 87.5%，特異度 = 96.7%)および5 mmHg以上の拡張期血圧低下(AUC ＝ 0.938, 感度 = 100.0%，特異度 = 88.6%)であることが示された。

During the head-up tilt test, eight and three participants showed systolic and diastolic orthostatic hypotension, respectively.
The receiver operator characteristic curve analyses revealed that the optimal cut-points for orthostatic hypotension using the sit-up test were a decrease of 10 mmHg in systolic blood pressure (area under the curve = 0.915, sensitivity = 87.5%, specificity = 96.7%) and 5 mmHg in diastolic blood pressure (area under the curve = 0.938, sensitivity = 100.0%, specificity = 88.6%).
