選択基準/Key inclusion criteria

日本語
膵胆道疾患（悪性・良性胆管狭窄、総胆管結石など）に対してERCPを受ける症例で以下のPEPのリスクファクターがあり、かつ膵管にガイドワイヤーが留置できた症例。
・過去の膵炎の既往。
・膵管造影を施行。
・Vater乳頭のプレカットを施行。
・胆管・膵管の精査（細胞診、ブラシ細胞診、生検、管内超音波）を施行。
・胆管にメタリックステントを留置。

The cases who were performed ERCP to investigate or treat pancreaticobiliary diseases. The patients has more than one of the below risk factors and are placed ERCP guidewire to main pancreatic duct.

1. Past history of pancreatitis
2. Endoscopic pancreatography
3. Precut of Vater papilla
3. The invasive pancreaticobiliary investigation (brush cytology, biopsy, IDUS, cygology of pancreatic juice, etc...)
4. The biliary metallic stent insertion

除外基準/Key exclusion criteria

日本語
（有効性評価に影響を及ぼすことによる除外）
・Vater乳頭に内視鏡的乳頭括約筋切開術やステント留置、内視鏡的乳頭バルーン拡張術などの処置
を施行されている。
・消化管狭窄があり、内視鏡の挿入が困難。
・胆管挿管の難易度が明らかに上がるような腹部手術の既往。
・膵管癒合不全。
・膵管内乳頭状粘液性腫瘍の症例。
・治療目的の膵管ステント挿入術。
・慢性膵炎、膵頭部癌症例。
・内視鏡的Vater乳頭切除術を施行された症例。
・膵胆管合流異常。

（安全性評価に影響を及ぼすことによる除外）
・ERCP時に使用する薬剤に対するアレルギーを有する。

（リスク／ベネフィットバランスの観点からの除外）
・ERCP前より膵アミラーゼが150 IU/L以上などの高値である場合や、急性膵炎に罹患している。
・十分に鎮静できず検査の続行が困難である。

7. Chronic pancreatitis, pancreatic head cancer

9. Malfusion of pancreaticobiliary ducts
10. Allergies to drugs that are used in ERCP
11. Acute pancreatitis, or serum hyperamylasemia (pancratic isozyme more than 150 IU/L)
12. Not sufficiently sedated
