選択基準/Key inclusion criteria

下記のような直腸ESDで一括切除が必要な患者
1)スネアEMRによる一括切除が困難な病変
・LST-NG, 特にpseudo-depressed type
・Vi型pit patternを呈する病変
・T1(SM)軽度浸潤癌
・大きな陥凹性腫瘍
・癌が疑われる大きな隆起性病変
2)粘膜下層に線維化を伴う粘膜内腫瘍
3)潰瘍性大腸炎などの慢性炎症を背景としたsporadicな局在腫瘍
4)内視鏡切除後の局所遺残腫瘍

The patients that needed rectal endoscopic submucosal dissection like below
1)a tumor that en bloc resection is difficult by endoscopic mucosal resection like below
LST-NG, especially pseudo-depressed type, the lesion that has a type of Vi pit pattern, shallow submucosal invasion, a large elevated lesion suspected cancer,
2)an intramural tumor with fibrosis of submucosal layer
3)a tumor caused by chronic inflammation such as ulcerative colitis
4)a residual tumor after endoscopic resection

除外基準/Key exclusion criteria

肛門管部の切除創面が全体の10%以上を占めると予測される直腸肛門腫瘍

Complete closure of the defect (median defect size 29.0 mm) was successfully achieved in 24 cases (80%). Delayed bleeding occurred in one case with incomplete closure (3.3%). The median ELOC procedure time was 25.5 minutes (interquartile range, 20.0 to 30.0 minutes). The sustained closure rates were 83.3% (20/24) on POD 3 in the 24 cases with complete closure.
