日本語
本研究の目的は、糖尿病の教育入院で実施されている構成要素を参考にしながら、在宅で同等の教育を実施しつつ行動変容を促すためのプラグラムを構築することである。また、医師の指示のもとプログラム内容が患者の病態を悪化させないという安全性も考慮しながら設計することも目的としている。

The purpose of this study is to build a program that brings change to one's behavior.
The program will refer to in-hospital diabetes educational program, and designed to be safe (not to worsen the patient's condition) under the direction of a doctor.

選択基準/Key inclusion criteria

日本語
1) 筑波大学付属病院に通院されている糖尿病をお持ちの方
2) 1)の方で直近の検査でHbA1cが7.0%-7.9%の方
3) 臨床試験開始時点で、年齢が満20歳以上65歳未満の方
4) 臨床試験開始時点で、SU薬の服用またはインスリン注射をしていない方
5) 1日6,000歩程度の運動（歩行）が可能な方
6) 通常の教育入院が困難な方

1) Diabetic patients of the University of Tsukuba Hospital
2) Patients whose HbA1c is more than 7.0 % and less than 7.9 %
3) Patients aged between 20 and 65 at the start of this clinical trial
4) Patients who are not taking sulfonylureas or injecting insulin at the start of this clinical trial
5) Patients who can walk about 6,000 steps a day
6) Patients who have difficulty in normal education hospitalization

除外基準/Key exclusion criteria

日本語
1) 同意取得時に、重症感染症、手術前後、重篤な外傷のある方
2) 運動制限のある方（腎機能低下（eGFR30未満)・活動性の網膜症（福田分類：悪性網膜症(B)相当）など）
3) 妊婦、授乳婦、妊娠している可能性または予定のある方
4) スマートフォンを所有していない方
5) その他主治医並びに研究担当医師が不適切と判断した方

1) Patients with severe infections, before or after operations or having serious external injuries at the time of agreement acquisition
2) Patients with exercise restrictions (reduced renal function (eGFR < 30), progressive retinopathy)
3) Pregnant women, lactating women, or women who have possibility of pregnancy
4) Patients who do not own smartphone
5) Patients who are judged to be inappropriate by the research managers
