レセプト及びDPCから構成されるデータベースを用いて、慢性透析患者において、下肢末梢動脈指導管理加算を算定している患者と算定していない患者で大切断の割合を比較検討する。

Using a database consisting of receipts and DPCs, we will compare the rate of major amputations between patients with chronic dialysis patients who have calculated guidance/management fee for peripheral arterial disease of lower limb and those who have not.

選択基準/Key inclusion criteria

下肢末梢動脈疾患指導管理加算が算定可能となった2016年4月をIndex Date（t0）とする。Index Date（t0）を起点として、以下の満たす患者
（1）t0時点で慢性透析療法を受けている患者
（2）t0時点で年齢20歳以上75歳未満

Index date(t0) is March 2016, when guidance/management fee for peripheral arterial disease of lower limb can be calculated. Patients who meet the following conditions starting from Index Date (t0).
(1) Patient receiving dialysis at t0
(2) Patients aged 20 to 75 years at t0

除外基準/Key exclusion criteria

日本語
（1）評価期間内に外傷又は悪性腫瘍により大切断に至った患者
（2）悪性腫瘍の既往歴のある患者（t0以前に悪性腫瘍に対する治療が実施されている）
（3）評価期間途中に下肢末梢動脈疾患指導管理加算を算定し始めた患者
（4）認知症患者（t0以前に、認知症の治療が実施されている）
（5）精神疾患患者t0以前に、精神疾患に対する治療が実施されている）
（6）研究対象期間（2012.4から2019.6）内に腎移植を実施した患者
（7）膠原病や血管炎等（希少疾病を含む）を合併している患者（t0以前に、膠原病や血管炎等に対する治療が実施されている）
（8）2016年1月から3月までに透析導入された患者
（9）補助人工心臓を使用している患者（t0以前に、補助人工心臓を装着している）
（10）EVT又はバイパス術を実施している患者（t0以前に、バイパス術又はEVTを実施している）

(1) Patients who have undergone major amputation due to trauma or malignant tumors within the evaluation period
(2) Patients with a history of malignant tumors (Treatment for malignant tumors has been performed before t0)
(3) Patients who started to calculate the guidance/management fee for peripheral arterial disease of lower limb during the evaluation period
(4) Dementia patients(Dementia treatment has been performed before t0)
(5) Patients with mental disease(mental disease treatment has been performed before t0)
(6) Patients who underwent kidney transplantation during the study period (April 2012-June 2019)
(7) Patients with collagen disease, vasculitis, etc. (including rare diseases) (collagen disease, vasculitis, etc. treatment have been performed before t0)
(8) Patients who started dialysis from January to March 2016
(9) Patients using a auxiliary artificial heart (using a auxiliary artificial heart before t0)
(10) Patients undergoing EVT or bypass surgery (undergoing EVT or bypass surgery before t0)

本研究は、2012年4月から2019年6月までに収集された医療情報データベースを活用した後向きコホート研究である。慢性透析療法を受けている患者を特定し、下肢末梢動脈疾患指導管理加算を算定している患者と算定していない患者の2群間で大切断の割合及び費用対効果を算出・評価する。

This study is a retrospective cohort study utilizing the medical information database collected from April 2012 to June 2019. Patients undergoing chronic dialysis therapy are identified, and the rate of major amputation and cost-effectiveness are calculated and evaluated between the two groups of patients who have calculated guidance/management fee for peripheral arterial disease of the lower limb and those who have not.
