目的1/Narrative objectives1

日本語
患者個々の血圧特性や心血管疾患発症をトリガーする要因を明らかにし、イベント発症を抑制に繋げることを大目的とする。
(1) マルチセンサーABPMを用いて、日本全国より複数回の測定データを収集し、生活環境・波形情報を含む全国血圧データベースを構築する。
(2) 本データベースを用いて、患者背景、及びABPMに基づく血圧変動を基に、個々の循環器リスク因子の同定、リスク予測、またABPM検査の特性について探索する。

The main purpose of this project is to clarify the blood pressure variability of individual patients and the factors which trigger the onset of cardiovascular diseases.
(1) We will collect multiple blood pressure measurements from all over Japan using a multi-sensor ABPM, and construct a nationwide blood pressure database including waveforms of ABPM and living environments.
(2) Using this database, we will identify individual cardiovascular risk factors based on patient background and blood pressure variability measured by ABPM, predict cardiovascular risks, and explore the characteristics of ABPM tests.

24時間血圧平均値と主要イベント発症との関連
主要イベント：総死亡、心血管死亡、突然死、虚血性心疾患（急性心筋梗塞、狭心症）、
脳血管障害（脳梗塞、脳塞栓、脳出血、くも膜下出血）の発症、大動脈解離、入院を要する心不全

Relationship between the averages of 24hr-ambulatory blood pressure measured by ABPM and the onset of major cardiovascular events.
Major cardiovascular events: total death, cardiovascular death, sudden death, ischemic heart disease (acute myocardial infarction, angina pectoris), cerebral hemorrhage, subarachnoid hemorrhage), aortic dissection, and hospitalization on by heart failure.

副次アウトカム評価項目/Key secondary outcomes

1) 24時間血圧平均値と下記の疾患発症との関連：
新たに症状が出現した心房細動の発症、末梢動脈疾患の発症、腎機能悪化（慢性透析の導入を含む）、新規糖尿病の発症、症候性痛風発作の発症
2) 各24時間血圧平均値、血圧変動及び他のリスク因子と心血管疾患発症との関連
3) 各24時間の血圧波形と身体活動度、温度など環境因子との関連

1) Relationship between the averages of 24-hr blood pressure measured by ABPM and the onset of the following disease:
New onset of atrial fibrillation, peripheral arterial disease, deterioration of renal function (including initiation of chronic dialysis), diabetes mellitus, or onset of a gout attack.
2) Relationship between the mean value of 24-hr blood pressure, blood variability, or other risk factors and the onset of the major cardiovascular events.
3) Relationship between each waveform of 24-hr blood pressure and the environment (cf. temperature) or lifestyle (cf. activity) factors.

次の1)2)の登録基準に該当する者
1) 心血管疾患*1の既往、心血管リスク因子*2または自律神経障害*3のうち１つ以上を有する患者
2) 20歳以上

*1: 脳血管障害（脳梗塞・脳塞栓・脳出血・くも膜下出血）心筋梗塞・狭心症・大動脈解離・末梢動脈疾患・心不全
*2: 糖尿病・高脂血症・高血圧（治療中高血圧または診察室血圧130/80 mmHg以上）・慢性腎臓病（CKD）・心房細動・睡眠時無呼吸症候群・慢性閉塞性肺疾患・高尿酸血症・肥満症・現在の喫煙
*3: パーキンソン病、レビー小体型認知症、脳血管性認知症

1) Patients with at least one of the following: history of cardiovascular disease*1, cardiovascular risk factors*2, or autonomic neuropathy*3
2) Age 20 years or older

除外基準/Key exclusion criteria

次の除外基準のいずれかに該当する者
1) 6ヶ月以内の脳血管障害（無症候性または一過性は含まない）の既往のある患者
2) 慢性透析中の患者
3) その他の重篤疾患（癌、膠原病など）を治療中の患者
4) 主治医がABPM検査実施に適さないと判断した患者

Patients who meet any of the following exclusion criteria:
1) Patients with a history of cerebrovascular disease (excluding asymptomatic or transient) within 6 months before enrollment
2) Patients with chronic renal failure undergoing hemodialysis
3) Patients undergoing treatment for other serious diseases (cancer, collagen disease, etc.)
4) Patients who are judged by the attending physician to be unsuitable for the ABPM test.

【研究方法】
登録後にマルチセンサー搭載携帯型自動血圧計(TM2441,A&D社製)を用いて、24時間血圧測定とその後5日間の家庭血圧を測定する。
以降、7年間の予後、処方箋、診察室血圧の追跡、及び同機種による24時間血圧測定結果を診療録より記録する。

Methods:
The participants will measure their 24-hour blood pressure and home BPs of the morning and evening for five days following the 24-hr BP using TM2441 (A&D) at the time of baseline. During the study period, the medical information, such as diagnosis of cardiovascular events, prescriptions, office BPs and 24-hour BPs measured by TM2441, will be collected from their medical records for a maximum of seven years.
