日本語
腹腔鏡下胃全摘術(LTG)におけるOverlap食道空腸吻合術(OEJ)は、LTG後の自動縫合器を使用した安全なかつ完全内視鏡下に施行可能な消化管再建法の1つである。OEJは、吻合部の縫合不全や狭窄の報告は非常に少なく、かつ食道の切除長が長い場合でも使用可能であり、優れた食道空腸吻合法であると考えられる。しかし、LTG後にOEJを行った患者の長期観察中には、吻合部狭窄を伴わない食道裂孔近傍における挙上空腸の屈曲によって通過障害を来たす患者を経験することがある。本合併症に対しては、原則として再手術による解除が必要となるが、OEJにおけるこのような吻合部狭窄を伴わない通過障害の報告はない。そこで、本合併症を発症した患者を抽出し、その患者因子や臨床病理学的因子、周術期因子、手術所見などを詳細に分析することで、本合併症の治療と予防の特徴を明らかにすることを目的とする。

Overlap esophagojejunostomy (OEJ) in laparoscopic total gastrectomy (LTG) is one of the gastrointestinal reconstruction methods that can be performed for safe and complete laparoscopic surgery using a linear stapler after LTG. OEJ is considered to be an excellent esophageal jejunal anastomosis method because there are very few reports of anastomotic leakage and stenosis, and it can be used even when the esophageal resection length is long. However, during long-term observation of patients undergoing OEJ after LTG, patients may experience passage obstruction due to bending of the elevated jejunum near the esophageal hiatus without anastomotic stenosis. As a general rule, this complication requires re-operation to relieve the stenosis, but there have been no reports of passage obstruction in OEJ without the anastomotic stenosis. Therefore, by extracting patients who have developed this complication and analyzing the patient factors, clinicopathological factors, perioperative factors, surgical findings, in detail, the characteristics of treatment and prevention of this complication can be identified. The purpose is to clarify.

2009年４月から2020年５月まで佐賀大学医学部附属病院　一般消化器外科で胃癌に対し、腹腔鏡下胃全摘術を行い、Overlap法で食道空腸吻合術を施行した患者かつ食道空腸吻合部に屈曲による挙上空腸での通過障害を来した患者。

From April 2009 to May 2020, patients who underwent laparoscopic total gastrectomy for gastric cancer at Saga University Hospital in department of Surgery and underwent esophagojejunostomy using the Overlap method.
Among them, patients who had an obstruction of passage in the elevated jejunum due to flexion at the anastomotic site of the esophagus and jejunum.

除外基準/Key exclusion criteria

日本語
定期サーベイランスから外れた患者。
食道切離長が4cm以上となり、胸腔内吻合を要した患者。

英語
Patients who are out of regular surveillance.
Patients who required intrathoracic OEJ through a thoracoscopic approach because they required excision of over 4 cm of esophagus.
