This was a retrospective chart review to evaluate a personalized predictive digital CD computational model. This study involved adult patients, diagnosed with CD and treated in a routine clinical practical setting for CD. The data of subject was collected from existing medical record, and therefore there was no additional observation for this study.

収集された83例のデータを分析した結果、有害事象の発現割合は27.7%(23/83)で、因果関係を否定できない有害事象の発現割合は2.4%(2/83)であった。主な有害事象(複数例発現)は上咽頭炎(8例)、CD(3例)、帯状疱疹、埋伏歯(各2例)であった。また、因果関係を否定できない有害事象は帯状疱疹、尿管結石症(各1例)であった。死亡例はなく、重篤な有害事象の発現割合は9.6%(8/83)で、内訳はCD(2例)、肛門皮膚垂、肛門狭窄、腸閉塞、大腸狭窄、敗血症、感染性腸炎、薬物過敏症、肺の悪性新生物、ラクナ梗塞(各1例)で、いずれも治療との因果関係は認められなかった。

As a result of analyzing data obtained from 83 patients, the incidence of treatment emergent adverse events (TEAEs) was 27.7% (23 of 83 patients) and the incidence of treatment-related TEAEs was 2.4% (2 of 83 patients). The most common TEAEs (occurring in multiple patients) were nasopharyngitis (8 patients), CD (3 patients), herpes zoster and tooth impacted (2 patients each). The treatment-related TEAEs were herpes zoster and ureterolithiasis (1 patient each). There were no occurrences of deaths. The incidence of serious TEAEs was 9.6% (8 of 83 patients), consisting of CD (2 patients), anal skin tags, anal stenosis, intestinal obstruction, large intestinal stenosis, sepsis, enteritis infectious, drug hypersensitivity, lung neoplasm malignant, and lacunar infarction (1 patient each). None of these events were related to the treatment.

Primary Endpoints:
1. Proportion of subjects with a digital twin match in the virtual patient library.
2. Proportion of correct predictions across biomarkers, clinical outcomes, endoscopic images and treatment sequence dimensions.
Secondary Endpoints:
1. Changes in the Crohn's Disease Activity Index (CDAI), Crohn's Disease Endoscopic Index of Severity (CDEIS) and Simple Endoscopic Score for Crohn's Disease (SES-CD) scores from baseline to 14, 26, 52 and 104 weeks after treatment initiation with either vedolizumab or ustekinumab.
2. Proportion of subjects achieving endoscopic mucosal healing, defined as absence of ulceration, at 14, 26, 52 and 104 weeks after treatment initiation with either vedolizumab or ustekinumab.
3. Proportion of subjects achieving endoscopic remission, defined as a SES-CD score of 4 points or less, at 14, 26, 52 and 104 weeks after treatment initiation with either vedolizumab or ustekinumab.
4. Proportion of subjects achieving endoscopic response, defined as a more than 50 percent reduction in the SES-CD score, at 14, 26, 52 and 104 weeks after treatment initiation with either vedolizumab or ustekinumab.
5. Proportion of subjects achieving clinical response, defined as a more than 100-point decrease in the CDAI score from baseline, at 14, 26, 52 and 104 weeks after treatment initiation with either vedolizumab or ustekinumab.
6. Proportion of subjects achieving clinical remission, defined as a CDAI score of 150 points or less, at 14, 26, 52 and 104 weeks after treatment initiation with either vedolizumab or ustekinumab.
