本研究の第一の目的は、「アドバンス・ケア・プランニング（Advance Care Planning:ACP）をしておかないと、家族の精神的負担が強くなる」こと（損失回避ナッジ）を伝えることで、ACP実施に対する意欲が向上するかを検証することである。

ACP実施に対する意欲：（代理決定者を決めておくことへの意欲「重い病にかかり、病気の進行などにより自分で自分のことを決められなくなることに備えて、代理決定者を決めておこうと思いますか？」：Likert scaleで６項目より選択（とてもそう思う、そう思う、どちらかといえばそう思う、どちらかといえばそう思わない、そう思わない、全くそう思わない）、②代理決定者とACPの話し合いをしておくことへの意欲「あなたが、重い病にかかり、意思決定ができないくらい病状が悪くなり、回復不可能である時にどのような治療やケアを受けたか/受けたくないかについて、代理決定者その理由も含めて具体的に話し合っておきたいですか？」：Likert scaleで６項目より選択（とてもそう思う、そう思う、どちらかといえばそう思う、どちらかといえばそう思わない、そう思わない、全くそう思わない）。

Willingness to implement ACP
(1) Willingness to decide on a surrogate decision-maker: "Do you want to decide on a surrogate decision-maker in case you have a serious illness and cannot decide for yourself due to the progression of the illness or other reasons?

2) Willingness to discuss ACP with the surrogate decision-maker: "Would you be willing to discuss specifically what kind of treatment and care you received/would not receive when you were seriously ill, so ill that you could not make decisions, and unable to recover, including the reasons for this? (Select from 6 items on the Likert scale: very much agree, agree, somewhat agree, somewhat disagree, disagree, or not at all agree).

① がん患者もしくはがん体験者であること
② 年齢が40歳以上であること
③ 信頼できる家族や友人等がいること
④ 日本語によるコミュニケーションが可能であること
⑤ 文書で同意が得られること

(1) Must be a cancer patient or cancer survivor
(2) Must be at least 40 years of age
(3) Have a trusted family member or friend
(4) Able to communicate in Japanese
(5) Consent must be obtained in writing.

除外基準/Key exclusion criteria

② 信頼できる家族や友人がいない方
