肝外胆管癌と確定診断された症例を対象に放射線科医による読影実験、物理評価を行い、Dynamic-CT上の所見を病理所見と対比することで、胆管癌の評価における適切なプロトコールを後ろ向きに探索する。

To retrospectively explore appropriate protocols in the evaluation of extrahepatic cholangiocarcinoma for subjective assessment and diagonostic performance by radiologists in cases with a confirmed diagnosis of cholangiocarcinoma, and contrasting findings on Dynamic-CT with pathology findings.

主要アウトカム評価項目/Primary outcomes

・胆管癌の原発部位（肝門部の場合はBismuth分類も評価する）
・各撮影相における水平進展の評価
・水平進展の方向性(十二指腸側、肝側)
・各造影相における血管浸潤の評価
・各撮影相における隣接臓器浸潤の評価（肝・膵・十二指腸）
・各造影相における造影効果の主観的評価（5段階評価）
・各造影相における物理評価（CT値の測定）
・各造影相でのScan delay ratioの測定

Primary site of cholangiocarcinoma (if in the hilar region, also evaluate the Bismuth classification)
Evaluation of horizontal extension in each imaging phase.
Direction of horizontal extension (duodenal or hepatic).
Evaluation of vascular invasion in each contrast-enhanced phase.
Evaluation of adjacent organ invasion in each imaging phase (liver, pancreas, duodenum)
Subjective assessment of contrast effect in each contrast phase(5-point scale).
Objective assessment in each contrast phase(measurement of CT values).
Measurement of scan delay ratio in each contrast phase.

選択基準/Key inclusion criteria

・2011年1月1日から2021年12月31日までに岩手医科大学附属病院または附属内丸MCにおいて手術が施行され病理的に胆管癌の確定診断がされた症例。
・治療前(胆管ステントや外科的切除、化学療法)にCTを撮影した症例。
・術前・術後を問わず、病理学的検査で胆管癌と診断されている。
・年齢は20歳以上。
・性別限定なし。

Patients who underwent surgery at Iwate Medical University Hospital or affiliated Uchimaru MC between January 1, 2011 and December 31, 2021, and had a pathologically confirmed diagnosis of cholangiocarcinoma.
Cases in which CT was taken before treatment (bile duct stent, surgical resection, chemotherapy).
Patients diagnosed with cholangiocarcinoma by pathological examination, whether preoperative or postoperative.
Patients must be at least 20 years of age.
No gender restriction.

除外基準/Key exclusion criteria

日本語
・CTが撮影されているが、撮影タイミングなどのプロトコル記載がない症例。
・臨床診断が肝外胆管癌ではない例。
・腎機能低下が確認された症例（eGFR 45mL/min/1.73m2未満）。
・オプトアウトにより参加拒否を申し出た症例。

Cases in which CT was taken but no protocol was described, such as the timing of the CT.
EHD cancer was not the clinical diagnosis.
Cases with confirmed reduced renal function (eGFR < 45 mL/min/1.73 m2).
Patients who refused to participate in the study by opting out.
