To verify the effect of rehabilitation inter-hospital cooperation system using needs and resources (university hospital, regional hospital, Community Based Rehabilitation Support Center, rehabilitation hospitals nursing home, and outpatient clinic) matching database, or "Reha Navi Tokyo".

1. Person with first stroke diagnosed by image findings and clinical examination.
2. Person who started rehabilitation program in the acute hospital.
1. Person with reattack of stroke.
