Objectives

Narrative objectives1

In this study, we will examine the expression levels of a series of bioactive substances such as inflammatory cytokines in carotid plaque of patients undergoing carotid endarterectomy. In addition, we will assess the relationship of pro-inflammatory/anti-inflammatory phenotypes of carotid plaque with glucose metabolism and pro-inflammatory/anti-inflammatory phenotypes of circulating monocytes, which will help us to develop new strategies for the prevention and control of atherosclerotic cardiovascular diseases.

1. Outpatients undergoing carotid endarterectomy in the Department of Neurosurgery of our hospital
2. Patients who are able to attend the hospital
3. Age: 20 years of age and older, and below 80 years old
4. Patients providing written informed consent

1. Subjects who have been determined to be unsuitable for the attending physician

This study is prospective cohort study. We will investigate the patients undergoing carotid endarterectomy from March 1st, 2011, to February 29th, 2014, in the Department of Neurosurgery of Kyoto Medical Center, who meet the selection criteria and whose informed consent is obtained. In this study, the expression levels of a series of inflammatory/anti-inflammatory mediators (TNF-alpha, IL-6, IL-10, MCP-1, TREM2) will be quantified in carotid plaque obtained from surgical patients undergoing carotid endarterectomy by real-time PCR, and the protein levels of inflammatory/anti-inflammatory mediators in blood samples will be determined by ELISA.
In addition, anthropometric and metabolic parameters will be measured (height, body weight, waist circumference, body mass index, systolic and diastolic blood pressures, HbA1c, FPG, TG, TC, LDL-C, HDL-C, UA, BUN, Cre). Circulating monocyte fractions will be isolated from the peripheral blood samples by MACS and subjected to the measurement of gene expression of the inflammatory/anti-inflammatory mediators (TNF-alpha, IL-6, IL-10, MCP-1, MHC-II, CD86, Dectin-1, CD206, CD16, CD68, TREM2).
These studies will reveal the mechanisms underlying the development of atherosclerotic plaque in patients with carotid stenosis and/or stroke, which will help us to develop new strategies for evaluating the progression of atherosclerosis and the postoperative prognosis.
