平成19年6月1日より平成19年11月30日の間、広島大学病院中央点滴室において外来化学療法を受けるがん患者で、言語的意思疎通が可能であり、研究への同意が可能な日本人とする。

除外基準/Key exclusion criteria

明らかな認知障害や視力障害、聴覚障害などつらさと支障の寒暖計に回答することが困難なものは対象から除外する。

主な結果/Results

日本語
がん患者において抑うつは看過ごされやすいことが示唆されている。本邦における抑うつの簡便なスクリーニング法として、つらさと支障の寒暖計が開発され、入院患者において精神科受診につなげるスクリーニング法としての有用性が示唆されている。外来化学療法を受ける患者は嘔気や食欲不振など重篤な副作用のために抑うつを潜在的にかかえやすいと考えられている。したがって外来診療において抑うつを察知し精神科診療に結びつける有効なシステムを開発する必要がある。本研究の目的は、患者にとって最も身近な医療者である看護師によるつらさと支障の寒暖計を用いた治療導入システムを開発することである。平成19年6月末より半年間に広島大学病院において外来化学療法を受けるがん患者を対象とし、結果130名の同意者に調査を行った。つらさ4点、支障3点の閾値以上であった38名のうち、精神科医による面接を希望したものは6名であった。不同意の理由は「まだ自分でなんとかできる。」「友人がいるから大丈夫。」「そこまではしなくても。」といったものであった。精神科医による診断の結果、大うつ病2名、適応障害4名であり精神科での治療継続を推奨し、5名が受診に至った。面接を希望しなかったもののうち、半数はその後の寒暖計調査で閾値を下回った。面接調査への満足度は平均78.3%（標準偏差14.7%）であり、低くはないため、対象者において精神的支援の潜在的ニーズはあり、一部の対象者には介入効果があったと考えられる。精神科受診への心理的ハードルを下げるとともに継続したサポート体制を構築することが課題である。
