日本語
培養細胞やモデル動物を用いた多くの基礎研究から動脈硬化（プラーク）の進展にマクロファージやケモカインなどの重要な因子が報告されている。しかしながら、実際にヒトの冠動脈病変部においてそれらの因子が発現し、影響を与えているか否かを検証することは移植例や剖検以外では困難である。そのため、従来の多くの臨床研究では、IVUSや血中の炎症マーカー等の測定のみで動脈硬化の進展、ACSの発症に関連する因子の解析が行われている。
経皮的冠動脈形成術（PCI）は冠動脈疾患の治療の中心を成すものであり、我々の共同研究施設合計でも年間約５５０例施行されている。このPCI施行時の問題点として、slow flow, no reflow現象が上げられる。これの現象はPCI施行時に血管内から剥がれたプラークや血栓が病変部のより末梢の細い血管を塞栓することにより発生し、最悪の場合冠動脈血流を失ってしまう。最近、この、slow flow, no reflow現象を予防するためのデバイスとしてPercuSurge（保険診療適応）が用いられるようになった。 PercuSurgeではPCI施行時に病変から剥がれた組織片,血栓や病変部周囲の血液を吸引カテーテルにより回収することにより末梢の細い血管の塞栓症のリスクを減らすことが可能である。通常、このPercuSurge時の吸引物は廃棄されているが、Filter basketに通すことによりプラーク組織片を得ることができる。更に、我々はfilterを通した血液あるいはカテーテルからの血液からCD14抗体を用いて単球・マクロファージを単離しRNAを抽出する手法を確立した。
そこで、我々はこのシステムを用いて降圧薬であるアンギオテンシン受容体拮抗薬（ARB）や抗高脂血症薬スタチン（Strong Statin, PPAR ligandsなどの使用により、サンプル内の動脈硬化関連遺伝子、及び蛋白発現、血清中の動脈硬化関連マーカー（cytokines, adipokinesなど)に差が生じるか否かの検討を計画している。
本研究の目的は、このシステムを用いて、１)安定狭心症とACS, 2)ARBやstrong statin, PPAR ligandsの使用の有無などの違いにより、実際にヒト病変部プラーク及び血液中から単離した単球・マクロファージにおいて動脈硬化関連遺伝子の発現に差異が認められるか否か、DNA arrayおよび定量的RT-PCRを用いて検討することである。

To identify the pathophysiology of atherosclerosis, we plan to analyze gene expression in monocyte isolated from patient with ischemic heart disease. We also plan to compare the difference of 1) gene expression, 2) serum cytokine level and 3) adipokine level before and after treatment of angiotensin receptor blocker, statin, PPAR ligands.

虚血性心疾患のために経皮的冠動脈形成術が必要な高血圧患者

Inpatient with acute myocardial infarction or angina pectoris who needs treatment of catheter intervention. And treatment of hypertension.

除外基準/Key exclusion criteria

同系薬物に対しアレルギーの既往がある患者
妊娠中あるいは妊娠の可能性のある女性患者
消化器疾患、重篤な肝障害のある患者（正常値の２倍以上）
悪性疾患、膠原病、感染症等の炎症性疾患
