主な結果/Results

高齢の非代償性急性心不全患者の院内死亡を規定する臨床マーカーを検討。
65歳以上の急性心不全患者349例の臨床検査値で院内死亡を独立して規定する因子として、Ｎａ≦138mmol/L,BUN≧35mg/dl,アルブミン≦3.2g/dl, BNP≧980pg/mlが明らかとなった。中でも低アルブミンが最も強力な予後規定因子であった。

Five independent prognostic factors were identified by multivariate logistic regression analysis, and each factor was assigned a number of points proportional to its regression
coefficient: prior heart failure hospitalization (2 points), sodium lower than138mmol/L (2 points), BUN greater than 35mg/dl(2 points), albumin lower than 3.2g/dl (3 points), and BNP greater than 980 pg/ml (2 points); in particular, hypoalbuminemia was
identified as the strongest prognostic factor.
