目的1/Narrative objectives1

日本語
半側空間無視は主に脳血管障害後に見られる重篤な認知機能障害であり、その病態は未だ不明な点が多い。診断については図形の模写や線分二等分試験といった古典的な検査が行われており規格化された定量的な評価法は確立されていない。治療法については磁気刺激やプリズム適応を用いた手法などが現在模索されている段階である。Acetylcholinesterase inhibitor (donepezil) は認知機能を高める薬理作用が実証されており、現在アルツハイマー型老年認知症に対してわが国を含め世界的に広く治療に用いられているが、脳血管障害による認知機能障害に対して治療効果があるかは検討されていない。
　本研究では(1)コンピューター化された視覚刺激検出課題を用いて半側空間無視の定量的評価法を確立し、(2)課題遂行中の脳波(event-related potentials, ERP)を評価することによって半側空間無視の病態生理を検討する。さらに注意、覚醒との関与が知られているアセチルコリンの半側空間無視に及ぼす効果を検討するためにdonepezilを投与したときの視覚刺激検出課題の成績を評価し、薬剤による半側空間無視の治療効果を検討する。

Hemispatial neglect (HSN) is a devastating cognitive dysfunction mainly caused by cerebrovascular accidents and its pathogenesis is still not well understood. Diagnosis of HSN is usually made based on figure copying and line bisection tests, which are not standardized. Several kinds of treatments, including extracranial magnetic stimulation and prism adaptation, remain explanatory. The present study aims to establish a quantifiable and standardized diagnostic method by using a computer-based visual detection task, and evaluate the effect of donepezil, widely used to treat Alzheimer disease, on the hemispatial neglect.

脳血管障害により半側空間無視を呈する患者で脳梗塞発症後１ヶ月以上経過した成人、および年齢をマッチした健常被験者。

Patients of hemispatial neglect at least 1 month after the onset of cerebrovascular accident. Age-matched healthy volunteer

除外基準/Key exclusion criteria

明らかな失語症状、認知症を伴う症例。てんかんの既往がある症例。

Those who present apparent aphasia or dementia. Patients with history of epilepsy.
