日本語
東洋古来の伝統医療である鍼治療は、単に局所の反応だけでなく、主に高位中枢を介して自律神経や免疫・内分泌機能などの反応が関与し、数多くの疾患や症状の改善に寄与しているという理念のもとに、我々は、鍼治療が各種生体機能や主に疼痛性疾患に及ぼす影響を研究してきた。これまで、一次性頭痛である緊張型頭痛の発症機序や鍼治療の作用機序について、plethysmographyやEMG、thermography、open loop video pupillographyを用いて検討した結果、頭痛の発症機序は、頭部の筋群よりも後頸部や肩甲上部・肩甲間部の筋群の過緊張が重要な役割を果たし、鍼の作用機序はこうした筋群の過緊張を緩和し、循環動態を正常化することにより頭痛の改善に寄与していることがわかった1,2,3,4)。また、こうした鎮痛機序は単に局所の反応(軸索反射)のみならず高位中枢(Edinger-Westphal核・中脳中心灰白質)に影響を及ぼし、自律神経系が重要な役割を果たしていることを明らかにした。さらに、緊張型頭痛患者と健康成人の鍼刺激による生体反応を比較した結果、患者と健康成人に及ぼす影響は異なり、鍼刺激はホメオスターティックな反応であることも分かった。
そこで今回は、鍼治療が健康成人の脳血流量に及ぼす影響を、安全で非侵襲的で反復検査が可能であるASLMRIを用い、脳血流量変化を鍼治療前後で比較し、鍼治療が高位中枢に関与する機序を明らかにする。

Under the concept that acupuncture being one of the traditional Asian medicines fostered over thousands of years contributes to alleviation of many diseases and symptoms not only through mere local reactions but also through the reactions of autonomic nerve system mediated by the high-level central nervous system and immunological/endocrine functions, we have investigated the influences of acupuncture on various body functions and on mainly painful affections.
Then, in this study, for the purpose of clarifying the mechanism of acupuncture against migraine, we will investigate the influences of acupuncture on the cerebral blood flow at the pain-sensitive sites of thalamus, hypothalamus, periaqueductal gray matter, ceruleus nucleus, nucleus raphe magnus, etc., which are considered to be the migraine-inducing organs, by determining the cerebral blood flow changes using Arterial Spin Labeled (ASL) as an index.

主な結果/Results

鍼刺激前の片頭痛患者の脳血流量は，健康成人と比 較した結果，後頭葉と右側頭葉で高く，左側頭葉と頭 頂葉楔前部で低下していた。鍼刺激前と比較し、健康成人は，視床や視床下部お よび弁蓋部や帯状回，島などの血流が，鍼刺激中 5 分・10 分後は増加した.

Before acupuncture stimulation, cerebral blood flow in migraineurs was higher in the occipital lobe and right temporal lobe, and lower in the left temporal lobe and precuneus of the vertebral lobe, compared with healthy adults. Compared to healthy adults before acupuncture stimulation, blood flow in the thalamus, hypothalamus, valgus, cingulate gyrus, and insula increased after 5 and 10 minutes during acupuncture stimulation.
