糖尿病や閉塞性動脈硬化症に起因する慢性下肢潰瘍治療において、骨髄炎の有無、範囲を正確に評価することは治療戦略上極めて重要である。従来骨髄炎はレントゲン、MRI、ガリウムシンチで評価されてきたが、いずれも早期発見や骨髄浮腫との鑑別、局在診断が困難などの問題点があり骨髄炎診断のスタンダードとはなり得ていない。当科ではガリウムシンチとSPECT-CTの融合画像構築による骨髄炎の局在診断と炎症活動性の定量評価を試みる。

Presence of osteomyelitis may result in major limb amputation in patients with diabetes gangrene and/or refractory chronic ulcer caused by peripheral arterial disease (PAD). X-ray, computed tomography, magnetic resonance imaging and 67Ga-Citrate scintigraphy can all sometimes detect osteomyelitis, but none has become the standard. We use 67Ga-citrate hybrid SPECT-CT fusion imaging to determine exact locations and quantitative severity of osteomyelitis.

本研究では、定量的ガリウム(Ga)-single photon emission computed tomography(SPECT)/CTによる下肢骨髄炎(lower limb osteomyelitis: LLOM)の骨切り術の適応判定を目的とした。骨切り術の実施率は、IBR＞8.4(71.4％)の患者が、IBR≦8.4(5.5％)の患者よりも高率であった(p＜0.001、感度：0.89、特異度：0.84)。多変量Cox回帰分析では、IBR＞8.4は骨切りの独立危険因子であった(ハザード比［HR］：19.0、95％信頼区間［CI］：5.6-63.9、p＜0.001)。

The aim of this study was to determine the indication for osteotomy in lower limb osteomyelitis by quantitative gallium-single photon emission computed tomography-CT. The rate of osteotomy was higher in patients with inflammation-to-background ratio (IBR) over 8.4 than in those with IBR less than 8.4. Multivariate Cox regression analysis showed that IBR over 8.4 was an independent risk factor for osteotomy.

本試験は、単施設の前向き診断試験である。もともと高気圧酸素療法の効果を評価した先行研究のマスターレジストリに登録されていた患者群において、少なくとも1つの慢性疾患（2型糖尿病、心血管疾患の既往、四肢虚血、血液透析の必要性など）を有し、2012年1月から2017年7月までに足潰瘍または安静時下肢痛を発症した患者を募集した。その中で、下肢骨髄炎が疑われる患者を本研究に登録した。

This was the single-centre prospective diagnostic study. In the group of patients who were originally registered in the master registry of our prior study evaluating the effects of hyperbaric oxygen therapy, we recruited consecutive patients who with at least one chronic disease (e.g., Type 2 diabetes, history of cardiovascular disease, limb ischemia, or requirement for haemodialysis) who developed foot ulcers or rest leg pain between January 2012 and July 2017. Among them, patients with suspected lower limb ischemia were enrolled in the study.

初回骨切り術は本試験の主要評価項目であった。本研究では、骨切り術は足関節以下の感染した骨の部分的な除去を意味し、膝下または膝上切断のような大きな四肢切断とは区別された。患者は、最初の67Ga-SPECT/CT検査から5年以上、同じ病院で外来観察を受けた。

Initial osteotomy was the primary endpoint of the study. In this study, osteotomy meant partial removal of infected bone below the ankle joint and was distinguished from major limb amputation such as below-knee or above-knee amputation. Patients underwent outpatient observation at the same hospital for more than 5 years after the first 67Ga-SPECT/CT examination.

クエン酸ガリウムSPECT-CTを、慢性骨髄炎が疑われる下肢潰瘍患者に施行する。治療前後でガリウムシンチ集積のTarget to Background Ratioを比較する。外科的切除が必要となるかどうかの閾値を設定する。

67Ga-citrate hybrid SPECT-CT will be performed to patients who are suspected having osteomyelitis, Pre- and Post-Target to Background Ratio (TBR; the accumulation at osteomyelitis site divided by that at normal distal end of the femur will be compared. The obtained data will show us the threshold whether the osteomyelitis site should be surgically removed or conservative treatment would work.
