これまで、器質的心疾患を有する成人に対して右室ペーシングは心不全(HF)のリスクを増やすと報告されている。器質的心疾患を有さない成人に対する右室ペーシングの影響ははっきりせず、ペーシングによる同期不全によるものか、元々ある器質によるものかはっきりしてしない。リウマチ因子が陽性患者、抗核抗体(ANA)陽性の母親から生まれた患者に自己免疫性心筋症がみられ、ANAの存在がペースメーカー患者に悪い影響を及ぼすと報告されている。また、米国ではANA陽性でペースメーカー植込み後の先天性完全房室ブロック患者は心機能が低下すると報告されている。しかし、日本人における右室ペーシング患者におけるANAの存在と心機能の関係は検討されていない。この臨床試験では、右室ペーシング患者におけるANAの存在と心機能の関係を検証することを目的とする。

Right ventricular (RV) pacing is associated with increased risk of heart failure (HF) in adults with structural heart disease (SHD). It is unclear whether this propensity for deterioration in cardiac function results exclusively from pacing-induced ventricular dyssynchrony or from the interaction of pacing with an abnormal substrate. Immune-mediated cardiomyopathy has been recognized in patients with a positive rheumatoid factor test result and in infants born to mothers with antinuclear antibodies (ANA). In the U.S., it has been reported that ANA positive patients with congenital complete atrioventricular block after pacemaker implantation decrease cardiac function. However, the relationship between the presence of ANA and cardiac function in patients with right ventricular pacing in the Japanese population has not been studied. In this clinical trial, with the aim of verifying the relationship between the presence of ANA and cardiac function in patients with right ventricular pacing in Japanese.

ペースメーカー、植え込み型除細動器(ICD)を移植され、右室リードが留置された患者で、横浜市立大学附属病院においてフォローアップされているもの。
