糖尿病遠隔診療での尿検査の試行
AUTION SCREEN Microalbumin/Creatinine（Arkray, 京都）ウロピースS（協和メデックス，東京）の試験紙を台紙と共に郵送。写真をe-mailにて添付送付。NIHimage(NIH, USA)により色見本と比較定量。事前のデータ検討では病院検査施設の定性結果とすべて一致。遠隔診療参加者10名のうち6名が尿検査。今回の対象者では再検結果では異常は改善。血液検査は全員が返送、尿検査での返送率は低かった。実際に可能であることが検証できた。

Urine examination under telemedicine in diabetic patients.
Among 10 patients enrolled, one dropped out, and six patients agreed urine examination. The results of urine examination may be reliable except for one unreliable report that sent examination of a wife of a patient.
It may be possible to carry out this kind of telemedicine in the treatment of diabetic patients, but the reliability of transmitted data should be well examined.

参加者の流れ/Participant flow

患者は埼玉医科大学総合医療センターの外来患者から募った。
検尿はAUTION SCREEN Microalbumin/Creatinine（Arkray, 京都）、ウロピースS（協和メデックス，東京）の試験紙を前者はパックのまま後者は乾燥剤を入れ患者に郵送し、筆者らが開発した台紙と共に郵送した。AUTION SCREENは微量アルブミンおよびクレアチニンの測定が可能となる。ウロピースSでは糖、蛋白、ケトン体が測定可能となる。台紙には色コントロールと時計を設置する場所を指定し全体を広く写真に撮るように説明をした。デジタルカメラやスマートフォンや携帯電話で写真を一定の反応時間の後（AUTION SCREENは60秒、ウロピースSは30秒）撮影しe-mailにて添付送付を指示した。データはNIHimage(NIH, USA)により色分析を行い定量化し色見本と数学的な関数を用いて比較定量を行った。定量したデータは色見本の判定に従い定性的な結果として報告した。

Subjects were recruited from the out-patients clinic of Saitama medical Center, Saitama Mddical University in Kawagoe-city.
Urine exam. was performed using pictures sent through WEB. Pictures of urine sticks were taken with standard panel that can be compared.

事前のデータ検討では病院の検査施設の定性結果とすべて一致していた。遠隔診療参加者10名のうち6名が尿検査に応じ１~3ヶ月ごとにe-mailにてデータを送付した。データはhttp://telemed.jpにて患者に閲覧可能とした（IDとパスワードが必要）。異常が検出された場合には再検を促した。今回の対象者では再検結果では異常は改善していた。

Accuracy examination of telemedicine was perfect, except for a pseudo sample. We conducted the study through WEB site.
All subjects resulted better HbA1c compared with HbA1c before.
