The number of subjects who moved to ESKD, initiated RRT, newly onset of CVD (cerebrovascular disease, coronary artery disease, and heart failure), and the number of death.

Psychological indicator: the self-efficacy scale on health behavior in patients with chronic disease.
The process indicators: the proportion of days in a month that subjects performed the following self-management behaviors.

選択基準/Key inclusion criteria

日本語
広島県呉市健康保険被保険者で、平成24年度及び平成25年度特定健診を受診した者（40歳以上）で、以下の基準を全て満たし、参加同意を得た者
適格基準
・尿たんぱく（２+）以上又は尿たんぱく(+)で15≦eGFR＜45
・糖尿病性腎症の診断がない

Participants are the beneficiaries of Kure-city national health insurance, aged over 40 years who received a specific medical check-up of 2012 and 2013, met criteria as described in the below, and provided informed consent to participate.
Inclusion criteria were
(1) urine protein level is higher than 2+, or
(2) urine protein level is 1+ and an estimated glomerular filtration rate (eGFR) between 15 to 44 mL/min/1.73 m2 at the registration point, and
(3) no diagnosis of diabetic nephropathy at the registration point.

除外基準/Key exclusion criteria

・透析導入、腎移植時
・全身状態悪化による入院等、主治医が研究続行不可能だと判断した場合
・死亡時
・認知症や精神疾患を有する者

Exclusion criteria were
(1) having introduced RRT,
(2) died,
(3) cognitively impaired or mental disorders, and
(4) when his/her primary physician judged that the enforcement of the program causes problems on the patients.
