急性胆嚢炎患者を対象として、標準治療である早期手術に対して、準早期手術が手術時間において非劣性であることを、ランダム比較試験によって検証する。

In the early group, patients undergo laparoscopic cholecystectomy within 72 h of admission.

介入2/Interventions/Control_2

EI群：準早期手術（抗生剤投与による炎症改善後7日以内）

In the early interval group, patients are treated with intravenous fluids and antibiotics until inflammation diminish (defined as the day of improvement). An elective laparoscopic cholecystectomy is scheduled within 7 d after the day of improvement.

（1） 急性胆嚢炎と診断された症例
（2）入院3日以内に腹腔鏡下胆嚢摘出術が施行可能な症例
（3）本研究への参加について、本研究登録前に患者本人による署名、日付が記載された同意文書が得られている症例

(1) Patients referred to our outpatient with a diagnosis of acute cholecystitis
(2) With a possibility of laparoscopic cholecystectomy within 72h after admission
(3) Written informed consent

除外基準/Key exclusion criteria

日本語
（1）重症度分類で重症と判断された急性胆嚢炎
（2）PS　ⅣかⅤ（ASA）
（3）胆嚢の穿孔、もしくは膿瘍形成が疑われる症例
（4）総胆管結石を有する症例
（5）上腹部手術の既往がない
（6）生命予後が48時間以内と見積もられる症例
（7）妊娠中や授乳中である症例
（8）抗生剤（ピペラシリン/タゾバクタム）に対してアレルギーがある症例

(1) Sever cholecystitis (Based on TG13)
(2) ASA (American society of anesthesiologists) physical status 4 and 5
(3) perforation or abcess of the gallbladder
(4) co-existing common bile duct stones
(5) previous upper abdominal surgery
(6) life expectancy less than 48h
(7) pregnancy or breastfeeding
(8) contraindications against the antibiotic (PIPC/TZB) used in this trial
