日本語
食道がん化学療法患者では，QOLの低下に影響を及ぼす有害事象として口腔粘膜炎を併発することが報告されている。一方、基礎研究において、漢方薬に口腔保健向上の可能性が見出された。そこで本研究では，有害事象予防としての漢方薬応用の有効性を調べることを目的とした。徳島大学病院にて初回DFP療法を行った食道がん患者40名を対象とした。対象患者の内訳は，コントロール群（17名：未介入）、専門的口腔ケア群（9名：専門的口腔ケアのみ），大黄甘草湯群（7名：専門的口腔ケア＋大黄甘草湯），半夏瀉心湯群（7名：専門的口腔ケア＋半夏瀉心湯）であり、口腔粘膜炎の発症状況，歯肉の状況，口腔細菌数および湿潤度を比較検討した。コントロール群と口腔ケア介入群との間で，重症口腔粘膜炎発症率に差があるか否かを分析した結果，専門的口腔ケア群では重症口腔粘膜炎未発症率は有意に低いことが確認できた。一方、口腔粘膜炎の予防に対する漢方薬の効果を認めることはできなかった。しかし，大黄甘草湯群ではその薬剤効果により，F. nucleatum 及び C. rectus歯周病関連細菌数の減少，歯肉の炎症を軽減することが示された。以上の結果から、漢方薬には口腔粘膜炎増悪時の口腔内細菌による感染拡大を防ぐ効果が期待されるため，食道がん化学療法患者への漢方薬を応用した口腔保健管理は有効である可能性が示された。
