急性心筋梗塞に対して可及的早期再灌流療法を施行された2 型糖尿病患者。

Type 2 diabetic patients undergoing primary percutaneous coronary intervention for myocardial infarction.

除外基準/Key exclusion criteria

日本語
1型糖尿病、冠動脈残存病変、弁置換後、重度弁膜症、腎不全、NYHA4度の心不全、重度高血圧、心房細動、肥大型心筋症、インスリン・グリニド薬の使用、ペースメーカー留置後、もともと心臓リハビリを受けている患者。

Type 1 diabetics, residual coronary stenosis, severe valvular disease, renal failure, NYHA class 4 heart failure, atrial fibrillation, hypertrophic cardiomyopathy, therapies with insulin, glinides, previous cardiac rehabilitation, and pacemakers.

主な結果/Results

日本語
当初予定した45名の被験者数には大幅に達していない現時点(n=14)での検定では、主要評価項目であるpeak VO2はプログラム前後で両群とも改善し（時間効果p=0.003、グループ×時間効果p=0.66）、その改善度は併用療法群で19±15%（p=0.02）、運動療法群で14±15%（p=0.03）であり、両群間に差は認めなかった。以上の結果より、DPP-4阻害薬の追加が運動耐容能に与える効果は少ないと考えられる。しかし、近年のDPP4阻害薬が広く使用されるようになりDPP4阻害剤を未内服の新規心筋梗塞患者は少ない。そのため、当初目標とした45例をエントリーすることは困難と判断し、本研究を中止する。
