目的1/Narrative objectives1

日本語
糖尿病網膜症、網膜静脈閉塞症や加齢黄斑変性症といった網脈絡膜疾患の診断には、蛍光眼底造影・インドシアニングリーン(ICG)眼底造影検査が必須であるが、造影剤を点滴静注するため、ショックやアレルギーという副作用や、腎機能障害症例では施行できない、という問題がある。一方で、光干渉断層計(OCT)は、網脈絡疾患の断層像を非侵襲的に観察することができ、現在の眼科診療においては、造影検査と並ぶ、必須の検査といえる。
従来のOCTでは、B―スキャン像（網膜面に対して垂直の断面像）の撮影を行っていたが、新しく開発されたEn Face OCTでは、C-スキャン像（網膜面に対して水平の断面像）も撮影することができ、さらに網脈絡膜各層の抽出ができる新しいテクノロジーである。Avanti OCT (Optovue社、米国)は、En Face OCTで、C-スキャン像が撮影できるのみならず、split-spectrum amplitude-decorrelation angiography (SSADA) アルゴリズムというソフトウェアを用いて、C-スキャン像から血流のある血管だけを描出し、あたかも眼底造影で得られるような画像(En Face OCT angiography)が観察できるようになった。造影剤を点滴静注しなくても網脈絡膜血管が観察できるというのは、副作用や侵襲を考えると、大きな利点といえる。
　今回の研究の目的は、従来の蛍光眼底造影・ICG眼底造影検査とEn-Face OCT angiographyの所見を比較し、En-Face OCT angiographyで、描出しやすい病変、描出しにくい病変、紛らわしい病変について明らかにし、将来的に造影剤を用いないEn-Face OCT angiographyのみで、網脈絡膜病変の評価を行うことができるかどうかを目的とする。

以下の基準を全て満たす患者を対象とする
1） 20歳以上の男女
2） 蛍光眼底造影・ICG眼底造影検査が可能な患者(施行済みあるいは施行予定)
　　3) Avanti OCT　検査が施行してあり、SSADA解析が可能な鮮明な画像が得られている患者

除外基準/Key exclusion criteria

以下のいずれかに抵触する患者は本試験に組み入れないこととする
1) フルオレセイン、インドシアニングリーン、ヨード等に重篤なアレルギーの既往を有する患者
2) その他、試験責任（分担）医師が患者として不適当と判断した患者
