【はじめに】長期血液透析患者の運動耐容能･歩行能力の低下はQOL低下、生命予後への影響が指摘されている（上月、2011）。透析患者を対象に、バランス練習アシスト（Balance Exercise Assist Robot：以下BEAR）のバランス機能改善効果を調べた。
【対象者と方法】運動療法の経験がない外来透析患者12名（年齢は71±8.5歳、男：女＝5：7名）。BEAR訓練（A）とエルゴメーター、静的・動的バランス練習からなる通常訓練（B）の交差試験とし、無作為にAB群とBA群に振り分けた。訓練はA、B各々16回、合計32回とした。1回2～3単位、週1～3回の透析日の透析前に実施した。評価項目はFunctional Reach test(FR)、10m歩行、Timed-Up&Go（TUG）、Berg Balance Scale（BBS）、FES-I（転倒セルフエフィカシー尺度）でA、Bの初回、中間、最終の計5回評価した。
【結果】脱落2例を除きAB群、BA群とも各5例であった。1例を除き9例の初期値に有意差がなかった。FR、歩行速度、BBSの運動機能とFES-Iが有意に改善した（Friedman test, p<0.01）。初回の介入時BEAR群は通常訓練群よりBBS改善度の差が有意であった（Mann-Whitney test, p=0.03）が、終了時には評価項目に差が見られなかった。
【考察】BEARは通常訓練より短期間に透析患者の身体運動機能、転倒不安定感の改善に有効であった。
