本研究で、周術期のEVAR後のAKI発症のリスク因子を後方視的に検討する。さらに、術後AKI発症患者と非発症患者のその後の死亡率、透析導入率に差があるかを比較検討する。

We seek the risk factors of postoperative AKI after EVAR. Moreover, we seek to determine if AKI was associated with an increase in all-cause mortality and incidence of postoperative permanent dialysis therapy among patients undergone EVAR.

Patients with thoracic or abdominal aortic aneurysm who underwent EVAR between 2010 and 2015 in Gifu University Hospital and Gifu prefectural general medical center.

除外基準/Key exclusion criteria

医療情報データベースから岐阜大学医学部附属病院と岐阜県総合医療センターにおいてEVAR手術を行った患者を対象とする。対象患者の年齢、性別、身長、体重、術前eGFR、術前T-bil、βブロッカー内服歴、ACEI内服歴、ARB内服歴、高血圧症
の既往、心不全の既往、糖尿病の既往、末梢動脈疾患の既往、COPDの既往、緊急手術、術中Flurbiprofenの使用、術中造影剤使用量、ASA分類、手術時間、麻酔時間、輸血量、輸液量、HES使用量をデータベースから抽出する。そして、それぞれの因子と術後AKIの発症に関連があるかを後ろ向きに検討する。また、術後死亡、術後新規透析導入の有無を電子カルテを用いて調査し、AKI発症患者と非発症患者で差があるかをCox multivariable regression analysisを用いて検討する。

The following characteristics are collected from clinical records; age, sex, height, body weight, preoperative sCr, preoperative eGFR, preoperative serum total bilirubin, coexisting hypertension, coexisting diabetes, coexisting chronic heart failure, coexisting chronic obstructive pulmonary disease, coexisting peripheral vascular disease, preoperative use of beta-blocker, preoperative use of angiotensin-converting enzyme inhibitor, preoperative use of angiotensin receptor blocker, emergency surgery, classification of American Society of Anesthesiologists, surgical duration, anesthetic duration, intraoperative amount of contrast administered, intraoperative amount of transfusion, intraoperative amount of fluid administered, and intraoperative amount of hydroxyethyl starch administered.
We seek the risk factors of AKI using Cox multivariable regression analysis.
