難治性慢性皮膚潰瘍に対してマゴットセラピーを施行することで、治癒の促進を図り、治療期間の短縮と患者QOLの向上を目指す。

To assess if the maggot therapy promote healing refractory chronic skin ulcer to shorten therapeutic time and improve QOL.

主要アウトカム評価項目/Primary outcomes

①局所所見：潰瘍は写真に撮影する。部位・深さ・壊死の形状を記録する。潰瘍の大きさは、長軸方向の最大長で評価する。足趾の間などの測定不能部位では面積を概算する。潰瘍の深さは、下記の基準による：１=表在欠損；２=皮下病変；３=腱もしくは骨の露出；４：腱もしくは骨の壊死、また、潰瘍は湿性か乾性であるかも評価する。②VAS（visual analog scale）による疼痛評価、③血液検査、④創部培養、⑤効果判定により、当研究が有効であり、しかも複数回の無菌マゴット投与が適応と判断された場合には、複数回の治療を行う、⑥潰瘍の感染がコントロールされ，潰瘍底の肉芽が良好となり潰瘍が全て上皮化する，あるいは植皮等の外科的処置を施行できる時点で投与終了とする。
上記評価を治療前、治療後2週、3週、4週、5週、6か月後に行う。

Those assessment will be performed before treatment and 2,3,4,5 weeks and 6 months after treatment.

選択基準/Key inclusion criteria

①糖尿病性壊疽・壊疽、②褥瘡、③下腿潰瘍（虚血性、うっ滞性、全身性強皮症をはじめとする膠原病など）

Untraceable chronic skin ulcer due to diabetic ulcer or diabetic gangrene, Decubitus, leg ulcer due to ischemia, stasis, and systemic sclerosis etc.
除外基準/Key exclusion criteria
日本語
・治療に対する同意が患者本人から得られない場合。
・著しい血流障害を伴いその改善が困難な場合。
・患部感染が急速に進み外科的切除の早期施行が必要とされる場合。
・膠原病に伴う潰瘍では、原疾患の病勢のコントロールが不良の場合。
・太い血管または消化管が露出している創部、体内深部に通じる創部。
・コントロール困難な出血性疾患を合併する場合
・過去に医療用ウジの使用で過敏症を起こした経験がある場合。
・その他、担当医師が被験者として不適当と判断した場合。

1. Patient who is not accepted the maggot therapy, 2. Severe blood flow obstruction, 3. Rapid exacerbated infection, 4. Poorly controlled collagen vascular disease, 5. Visible large vessels can be detected on the ulcer or connected to inside of the body, 6. Uncontrolled hemorrhagic disease, 7. Sensitivity for medical maggot. 8. In appropriate case judged by a doctor.
