目的1/Narrative objectives1

日本語
肝臓移植以外に治療法がない非代償性肝硬変患者に対して生体部分肝移植術後3日目に肝移植ドナー肝臓由来活性化ナチュラルキラー（NK）細胞を用いた免疫賦活療法を行うことで、肝臓移植後1カ月間の菌血症発現割合がヒストリカルデータに比べて低下することを非盲検試験にて検討する。

Incidence of blood stream infection during one month after liver transplantation

② 免疫応答への影響
CFSE-MLRによるドナー特異的免疫応答性の評価（術後1、2、3、4週）
DSA（donor specific antibody）の検出（年1回のスクリーニング）
拒絶反応発症率
レシピエント末梢血NK細胞活性評価
③ 肝細胞癌再発の有無（肝細胞癌症例のみ）
De novo発がんの有無、時期
④ 感染症発症率（細菌感染症、サイトメガロウイルス感染症、真菌感染症、遺伝子多型別解析）
⑤ 安全性評価（有害事象の種類と重症度、発現頻度、発現期間、因果関係）

選択基準/Key inclusion criteria

レシピエントの選択基準
1) 内科的治療抵抗性の非代償性肝硬変治療として生体部分肝移植術を施行する患者
2) 同意取得時年齢が20歳以上の患者
3) 本人もしくは代諾者の文書による同意が得られている患者

細胞提供者（ドナー）の選択基準
1) 肝移植研究会の「生体肝提供（ドナー）手術に関する指針」の基準を満たし、肝臓提供者として生体肝移植ドナー手術を受ける者
2) 同意取得時年齢が20歳以上の者
3) ドナー肝臓グラフトの灌流液から肝臓由来ナチュラルキラー細胞（NK）細胞を調製しレシピエントに投与することについて、本人もしくは代諾者の文書により、同意が得られている者

除外基準/Key exclusion criteria

1) 再肝移植を施行する患者
2) 脳死肝移植を施行する患者
3) その他、本臨床研究への参加を研究責任者又は研究分担者が不適当と判断した患者

細胞提供者（ドナー）の除外基準
1) 再肝移植に対する生体肝移植ドナー手術を受ける者
2) 本臨床研究への参加を研究責任者又は研究分担者が不適当と判断した者
