5 来院前12誘導心電図：記録、伝送
6 心筋梗塞形態：種類、部位、既往、peak CK、Peak CK-MB
7 治療：冠動脈形成術、血栓溶解療法
8 予後：入院中転帰、30日後転帰、死亡理由、12か月後予後、最終生存確認日、MACEの有無
