日本語
四肢、体幹の変形に対する矯正手術は一般的に2次元画像の単純X線を用いて変形量を評価し、術中は術者のマニュアル操作で矯正手術が行われている。そのため、３次元的矯正はもちろんのこと、２次元的な矯正も術前計画通り正確に行うことは不可能である。そこで我々は術前計画通り正確な３次元矯正手術を実現するために、患者個々の変形に適応したカスタムメイド手術ガイドを開発し、上腕骨遠位と橈骨遠位の矯正骨切手術に対するカスタムメイド手術ガイドの薬事承認が得られた。本研究の目的は上腕骨遠位、橈骨遠位以外の四肢、体幹の変形手術に対してカスタムメイド手術ガイドの適応を拡大し有効性を検討することを目的とする。

In the corrective osteotomy for the bone deformities, conventional preoperative planning with two-dimensional plain radiographs has not always provided sufficient information to understand the complex three-dimensional deformity. Surgeons has estimated the amount of reduction and manually corrected in the actual surgery. We have developed 3D computer simulations and a custom-made surgical guide to perform accurate corrective osteotomy. The custom-made surgical guide was obtained the regulatory approval in the distal humerus and the distal radius. The purpose of this study is to examine the surgical guide applying to limb bones and bony skeleton.

主要アウトカム評価項目/Primary outcomes

カスタムメイド手術ガイドを用いた矯正手術後のX線により術前のシミュレーションで予定していた矯正の誤差を計測することで、本臨床研究の目的である本術式の有効性を評価する。

四肢骨・体幹骨の変形（変形治癒骨折、骨端線障害・先天奇形に起因する骨変形）

Deformity of the upper extremity (malunited fracture, early closure of the growth plate, congenital anomaly)

除外基準/Key exclusion criteria

日本語
1.一期矯正が困難な高度の骨の短縮を伴う患者
2.全身麻酔または伝達麻酔が不適当な患者
3.妊娠中もしくは妊娠している可能性がある患者又は授乳中の患者及び本臨床研究中に妊娠を希望する患者
4.精神疾患により臨床研究の意義を正しく理解し、遵守事項を守ることができないと研究者が判断した患者
5.抗凝固薬の投与あるいは抗凝固療法を手術のために中止することができない患者

1. Sever shortening of the bone that cannot be corrected at one time.
2. Inappropriate candidate for general anesthesia.
3. Patient who is pregnant or possibly pregnant. Patient who hope to be pregnant during her trial.
4. Patient who has mental disorder and can not understand the significance of the study and preserve the compliance rules.
5. Patient who is administrated anticoagulant that cannot be interrupted for the operation
