体重減少率が胃全摘術を施行した場合より低いことを示すこと

QLQ-C30・STO22，PGSAS37における評価項目、体組成（脂肪量、筋肉量）、血液生化学的データ（血清タンパク、アルブミン、コレステロール、中性脂肪、リンパ球数）、無病生存期間、全生存期間、残胃癌発生率。

選択基準/Key inclusion criteria

(1) 病理組織学的に胃癌であることが確認されている。
(2) 術前検査にて、胃癌取扱い規約第14版におけるcStageIA（T1N0）もしくはcStageIB（T1N1、T2N0）と診断されており、胃切除術で治癒切除可能と判断される症例。なお、T2症例では大彎にかからない病変を適応とする。
(3) 先行してEMRやESDが行われていない場合、「cN1」あるいは「cN0」かつ、内視鏡検査にてEMRの適応外」と診断されている症例。内視鏡
検査におけるEMR（ESD）の適応外とは、以下のいずれかの条件を満たす場合を指す。
① 大きさが2 cmより大きい。
② 深達度が粘膜下層（SM）か固有筋層（MP）のいずれかと診断されている。
③ 主たる組織型が未分化型（por1、por2、sig、muc）である。
④ 肉眼型が0-II c 型（陥凹型）であり、かつ潰瘍もしくは潰瘍瘢痕を認める。
⑤ EMR（ESD）による腫瘍の一括切除が不可能と判断される。
(4) 先行してEMRやESDが行われている場合、EMR（ESD）後の病理組織学的検査により追加外科切除が必要※と判断される症例。
※ EMR（ESD）後の病理組織学的検査にて、以下のi)～vi）の少なくとも一つ満たす場合には追加外科切除が必要と判断する。また、EMR
（ESD）で分割切除となった場合、EMR（ESD）後の病理組織学的検査にて水平断端が陽性または判定不能だった場合に追加外科切除を行うかどうかは各施設の判断に委ねるが、追加外科切除が必要と判断された場合には適格に含める。上記以外の理由で追加外科切除を行う場合には適格とはしない。
i） 未分化型優位
ii） SM浸潤部に未分化型成分あり
iii） MかつUL（+）かつ3 cmを超える
iv） SM1かつ3 cmを超える
v） SM2以深
vi） 脈管侵襲陽性（ly とv のいずれか一方でも陽性）のもの
vii）垂直断端が陽性もしくは判定不能
(5) 食道への浸潤を認めない。
(6) 登録日の年齢が20 歳以上80 歳以下である。
(7) PS：Performance Status （ECOG）が0 または1 である。
(8) 上腹部手術の既往がなく、腸管（胃を含む）切除を伴う手術の既往がない。ただし、虫垂炎に対する虫垂切除は腸管切除に含めない。
(9) 他のがん種に対する治療も含めて化学療法（内分泌療法を含む）や放射線治療の既往がない。
(10) 自力で調査票の記載ができると判断される症例
(11) 本試験登録前に患者本人による署名、日付が記載された同意文書が得られている症例

英語
1) pathologically-confirmed gastric cancer (papillary adenocarcinoma, tubular adenocarcinoma, poorly differentiated adenocarcinoma, signet ring cell carcinoma and mucinous adenocarcinoma)
2) cStage IA or IB, but not T2 tumor at the greater curvature of the stomach
3) No indication of endoscopic mucosal resection (EMR) or endoscopic submucosal dissection (ESD) in a cN0 or cN1case
4) Patients who underwent EMR or ESD and need additional surgery
5) No invasion of the esophagus
6) Aged 20 to 80 year
7) PS (ECOG) 0 or 1
8) No history of gastrointestinal surgery
9) No history of chemotherapy or radiotherapy
10) Capability of filling out a survey independently
11) Provided written informed consent

除外基準/Key exclusion criteria

本試験の登録にあたっては、以下のいずれの基準にも抵触してはならな
い。
(1) 活動性の重複癌を有する症例※
※ 活動性の重複癌とは、同時性重複癌および無病期間が5年以内の異時性重複癌であり、局所治療により治癒と判断されるcarcinoma in situ（上皮内癌）もしくは粘膜内癌相当の病変は活動性の重複癌に含めない。
(2) 他の術式を同時に施行する症例（胆嚢摘出術等を除く）
(3) BMIが30を超える症例（BMI＝体重（kg）÷身長（m）2）
(4) その他、試験責任（担当）医師などが本試験を安全に実施するのに不適当と判断した症例

2) Simultaneous surgery other than cholecystectomy
3) Body mass index 30 or more
4) Any other medical condition that makes the patient unsuitable for inclusion in the study according to the opinion of the investigator
