目的1/Narrative objectives1

日本語
現在本邦において認知症患者は増加の一途をたどっており、２５０万人に迫る勢いであるといわれている。特発性正常圧水頭症は歩行障害、排尿障害、認知症状を三徴とし、認知症の原因疾患の一つである。日本においては約３１万人(人口10万人当たり約250人)の方が罹患していると推定されている。特発性正常圧水頭症は、脳室-腹腔シャント手術または腰椎-腹腔シャント手術などにより改善する可能性があるが、本疾患が認知機能に影響を及ぼす病態や改善の機序についてはいまだ不明である。一方、近年の脳機能研究の一つとして安静時機能的MRIを用いた脳機能ネットワーク解析がある。認知機能に関連するネットワークとしてはdefault mode network, salience network, executive control network等が挙げられるが、アルツハイマー認知症やmild cognitive impairment(MCI)を対象にした病態解明に関する様々な先行研究が報告がされている。本研究では、特発性正常圧水頭症患者の腰椎-腹腔シャント手術前後に機能的MRI（とくに安静時機能的MRI）画像所見の変化とともに認知症状の変化を比較し、シャント手術による脳機能ネットワークの変化の検証を非侵襲的に行うことを目的とする。アルツハイマーやMCIの先行研究と比較する事で認知症の病態や改善のメカニズムについての新たな知見を獲得できる可能性がある。

手術前、手術後2ヶ月における機能検査ならびにfMRI検査を行う。
高次脳機能検査（ACE-Ⅲ、MMSE、FAB、WAIS-R、ADAS、WMS-R、TMT）、運動機能検査(Timed Up&Go test)、機能的MRI検査

Patients with idiopathic normal pressure hydrocephalus. Patients with good enough cognitive function for the agreement of this study.

除外基準/Key exclusion criteria

閉所恐怖症、体内金属等でMRI検査の出来ないもの

Untolerable for MRI study.
