炎症性疾患(IBD)は近年増加の一途を辿っている。比較的若年者の患者が多く、この疾患の病勢が、進学、就職、結婚や出産などの人生における大きなイベントに影響を及ぼすことが少なくない。
IBDの診断には上下部消化管内視鏡、小腸内視鏡、カプセル内視鏡などを用いた内視鏡検査が必須である。またクローン病や腸管ベーチェット病など腸管に狭窄をきたし内視鏡が挿入困難な症例には内視鏡検査とともに消化管造影検査が重要な診断ツールとなっている。内視鏡検査所見や消化管造影検査、内視鏡検査で得られた生検病理所見及び臨床症状によってIBDの確定診断がなされている。また近年便潜血反応、便中カルプロテクチンなどが腸管炎症のバイオマーカーとして利用され、疾患活動性の指標となっている。
IBDの治療に関しては内視鏡で得られた腸管病変の重症度と下痢、腹痛といった臨床的重症度を鑑み、治療法が決定される。内科的治療としてはアミノサリチル酸製剤、ステロイド製剤、免疫調節薬、免疫抑制剤、生物学的製剤、血球成分除去療法、栄養療法、内視鏡的バルーン拡張などの治療を単独または併用して行い、また外科的治療として手術が選択される。
このように日常診療の中で用いられる内視鏡検査、消化管造影検査を用いた診断や各種治療法は日々進化を続けており、実臨床におけるこれらの診断・治療の価値、位置づけについては変遷することが予想される。そのため定期的に診断や治療法の有用性について評価を行うことはより良い医療を提供するためにも必要であり、また患者への治療に関する情報提供や将来のための治療成績の向上といった側面でも意義があると考える。
　今回我々は、日常診療にて行われたIBDに対する内視鏡検査、消化管造影検査を用いた診断や各種治療法の結果を後方視的に患者背景や臨床経過、病理像などと対比し、現在までのこれらの診断や治療の有効性を評価することを目的として本研究を行う。

Upper and lower gastrointestinal endoscopy, small intestine endoscopy and capsule endoscopy are essential for the diagnosis of IBD. Gastrointestinal contrast studies also become an important diagnostic tool. Findings of endoscopy and gastrointestinal contrast study, pathological findings obtained by biopsy and clinical symptoms make definitive diagnosis of IBD. In recent years, fecal occult blood test and fecal calprotectin are used as a biomarker of intestinal inflammation, which is an index of activity of the diseases.
The clinical severity of abdominal pain and diarrhea, the severity of the intestinal lesions obtained by endoscopy determine treatment of IBD patients. As a medical therapy, we can use amino salicylic acid, steroid, immunomodulator, immunosuppresant, biologics, cytapherasis, nutritional therapy and endoscopic balloon dilatation. Surgery is also chosen as surgical treatment.
It is necessary to evaluate the accuracy of diagnosis and the usefulness of these therapy, and we should improve medical care of IBD patients.

日常診療として診断、治療に用いられた個々の内視鏡画像、消化管造影画像、カルテ記録を整理し、患者さんの臨床経過とを対比することにより、炎症性腸疾患の診断や治療の有効性ならびに問題点を抽出することで、患者さんに研究のための負担をかけることなく、適切な診断方法や治療選択に結びつく有用な所見を見出すことを目的とする。
