目的1/Narrative objectives1

日本語
脳卒中・虚血性心疾患・心不全・腎不全を発症するリスクの高い生活習慣病に罹患している未受療者に対して、医療機関への受療行動とその継続を促進する「主治医と産業医の連携ツール」を用いて連携を強化した支援を職域で実施することは、一般的な定期健康診断事後措置としての受診勧奨による連携を実施するよりも、受療行動の継続および生活習慣病治療アウトカムやヘルスリテラシー（健康情報力）等関連項目に対する効果が大きいか否かを検証する

To investigate whether intensive health support using Contact Notebook between occupational physicians and clinic physicians in the workplace will encourage workers to visit clinics and to enhance the effect of treatment or the health literacy in high-risk individuals without clinical visits.

選択基準/Key inclusion criteria

① 労働安全衛生法に基づく定期健康診断の結果、
i) 血圧（収縮期160mmHg以上かつ拡張期100mmHg以上）
ii) 血糖（HbA1c(NGSP)7.0%以上（HbA1cの測定がない場合は空腹時血糖130mg/dl以上、空腹時血糖の測定がない場合は随時血糖180mg/dl以上）
iii) 脂質（男性のLDL-コレステロール200mg/dl以上
のいずれか一つ以上の項目に該当する人
② ①の健診受診時に①i、ii、iiiの該当する項目に関して医療機関を受療していない人

1.Participating in General Health Checkups held as the mass screening, according to the Industrial Safety and Health Law
2. Meeting at least one of the following criteria:
- systolic blood pressure >= 160 mmHg and dyastolic blood pressure >= 100 mmHg
- Hemoglobin A1c (NGSP) >= 7.0% (if hemoglobin A1c levels are missing, fasting glucose levels >= 130mg/dl; if fasting glucose levels are also missing, casual glucose levels >= 180 mg/dl)
- low-density lipoprotein cholesterol levels >= 200 mg/dl in males
3. Without clinical visits for hypertension, diabetes mellitus, and/or dyslipidemia

除外基準/Key exclusion criteria

研究協力者が定期健康診断の事後措置として労働者に受診勧奨の指導を行い医療機関の紹介を行う場合、その医療機関が、当該の労働者の所属する事業体が運営する等、同一の職場組織内にあると研究代表者または研究分担者が判断した場合。
