主な結果/Results

【結果】MOMM発生率は22.7%であった。ROC解析において手術開始前、終了時のPv-aCO2/Ca-vO2、ΔPv-aCO2/Ca-vO2に対するAUC (95%CI) は、0.536 (0.394-0.678)、0.780 (0.663-0.897)、0.666 (0.526-0.806)であり、手術終了時のPv-aCO2/Ca-vO2の最適閾値は1.3mmHg/mL (感度: 64.0% 、特異度: 71.8%)であった[Figure1]。多変量解析の結果、手術終了時のPv-aCO2/Ca-vO2はMOMM発生に対する独立した予測因子であった(Odds比: 8.05, 95%CI: 2.44-26.6, p<0.001)。

MOMM events occurred in 25 patients (22.7%). ROC curve analysis revealed that both postoperative Pv-aCO2/Ca-vO2 and Pcv-aCO2/Ca-cvO2 were significant predictors of MOMM. However, postoperative Pv-aCO2 was the best predictor of MOMM.Multivariate analysis revealed that postoperative Pv-aCO2 was an independent predictor of MOMM.

肺動脈-動脈二酸化炭素分圧較差/酸素含有量較差比の術後重症合併症に対する予測因子としての有効性をROC解析、ロジスティック多変量解析を用いて検討する。
術後重症合併症は以下のとおり定義する
a) 48時間以上の人工呼吸
b) 透析が必要な腎不全
c) 脳梗塞
d) 再手術
e) 深部創感染
f) 死亡

ROC analysis and logistic regression analysis will be conducted to investigate the predictive ability of the venous arterial CO2 to arterial venous O2 content difference ratio for severe postoperative complications.
Severe postoperative complications are defined as follows:
a) prolonged ventilation for more than 48hours
b) renal failure requiring dialysis
c) stroke
d) re-operation
e) deep sternal infection
f) death.
