近年注目されつつある障がい者に対する動物介在療法の中でも、歴史的に優れた社会復帰手段として知られる療法として乗馬療法(ホースセラピー)がある。本研究では、小児麻痺や脳卒中の後遺症として歩行障害を持つ患者を対象にホースセラピーを適応し、運動バランス解析や脳機能画像評価からその治療効果を明らかにすることを目的とする。

Hippotherapy is a treatment strategy that usesequine movement as part of an integrated intervention program for achieving functional outcomes. The aim of this study was to evaluate the effects of hippotherapy on temporospatial parameters of gait and balance abilities in patients with walking disabilities.

粗大運動能力尺度（GMFM）スコアによるホースセラピーの短期および長期改善効果。乗馬療法開始後の3次元加速度センサーを用いた携帯型歩行分析計による歩行およびバランス項目の経時推移 (5-mもしくは10-m歩行テスト); GMFMスコア成分(DおよびE)の変化。コントロール群(同時間のレクリエーション)もしくは年齢をマッチさせた正常データベースとの比較。

Short- and long-term improvement of hippothray assessed by Gross Motor Function Measure (GMFM).Temporospatial parameters of gait and balance abilities using a tri-axial motion recorder analysis for a 5-m or 10-m walikng test, score for dimensions D (standing) and E (walking, running, jumping) of the GMFM. Comparison was performed with control (recreation) or age-matched normal database subjects.

選択基準/Key inclusion criteria

日本語
既往疾患の種類に関わらず、乗馬施設における事前評価で乗馬療法の適応と判定された全患者。研究の目的、安全性について書面、及び口頭にて説明したのち、書面による同意書を得ることができたもの。

Children and adolescents with cerebral palsy in the Kanto region of Japan were recruited by contacting the schools that cater for them with appropriate physiotherapy services, as well as via adjacent hospital/clinics.

除外基準/Key exclusion criteria

上記の基準を満たさなかった全被検者。

Exclusion criteria included if they had 1) previous participation in hippotherapy, 2) history of fracture or epilepsy within 8 weeks of enrolment, and 3) cognitive or physical impairment sufficiently severe that would hinder study protocol. In addition, participants were excluded if they received medication or orthopedic surgery (e.g., botulinum toxin injection and selective dorsal rhizotomy) for treating tight muscles and spasticity related to CP during the study or within 3 months of the enrollment.
