①炎症性腸疾患(潰瘍性大腸炎、クローン病など)、家族性大腸腺腫症及びHNPCC(Lynch症候群)の患者
②前処置不良の症例
③ポリープなしの症例
④病変の病理評価が不能であった症例(resect and discard症例を含む)

日本語
JNET2B、3 LC症例は少なく、50名53個のJNET1, 2A LCポリープを解析対象とした。NBI拡大の正診率とNPVはそれぞれ58.5%(95%CI, 44.1%-71.9%)、66.0%(95%CI, 36.6%-77.9%)、Chromoendoscopy追加後 は66.0%(95%CI, 51.7%-78.5%)、61.1% (95%CI,43.5%-76.9%)であった。JNET分類を用いて腫瘍非腫瘍の鑑別がlow confidenceであった場合、追加したChromoendoscopyの結果に関わらず切除し病理提出が望ましい。

There were few JNET Type 2B or Type 3 with LC cases. Therefore, in 50 patients, we analyzed 53 polyps classified as JNET Type 1 or 2A with LC prediction. The accuracy and negative predictive value of magnifying NBI (first phase) were 58.5% (95%CI, 44.1% - 71.9%) and 66.0% (95%CI, 36.6% - 77.9%), and those of magnifying chromoendoscopy (second phase) were 66.0% (95%CI, 51.7% - 78.5%) and 61.1% (95%CI, 43.5% - 76.9%), respectively.
Regardless of the findings of additional chromoendoscopy, all polyps should be resected and submitted for histopathological examination when the confidence level in differentiating adenomatous from hyperplastic polyps is low by magnifying NBI.

佐野病院にて前向きに拡大大腸内視鏡を施行した患者に対して内視鏡的または外科的に切除したポリープに対しJNET分類(Type1,2A,2B,3)及びconfidence level（high,low）を記載した。Type2Bまたlow confidenceと診断した症例ではChromoendoscopy（インジゴカルミンもしくはピオクタニン染色）を追加し、追加した際の内視鏡診断の正診率、また実際にChromoendoscopyを要した割合について検討した。

This is a prospective study. Consecutive adult patients who underwent a colonoscopy with magnification in Sano hospital were recruited. The diagnosis of each polyp was evaluated with JNET classification and a level of confidence was described.
If the lesions were type 2B or any types with low confidence, additional chromoendoscopy (indigo carmine or crystal violet) was performed. We analysed the accuracy of additional chromoendoscopy in such cases and the proportion of cases which need additional chromoendoscopy.
