日本語
回答が得られた1型糖尿病55例、2型449例、その他の糖尿病53例を解析対象とした。食事の不足や内容の偏りが46.4％に、水分不足は16.2%に、運動量の減少は36.7%に、睡眠の不足は76.9%に認められた。医療情報は69.5%がテレビ・ラジオから、20.6%が携帯電話等の通信機器を介して得ていた。ストレス尺度として採用したIES-R値は、年齢、ライフラインの復旧の遅れ、糖尿病薬の自己判断での変更、及び運動量の増加が正に、ライフラインの復旧が2週間以内、睡眠の充足が負に関連していた。またIES-Rとアンケート項目の関連を、ポリコリック相関係数を指標とし、グラフィカルモデルにて相関係数を表したところ、地震の影響の受け方が病型によって異なることが示唆された。
1型糖尿病においてはHbA1c、グリコアルブミンともに有意な変動を認めなかった。1型では7％がインスリン増量、20％が減量しており自己調整が奏功したと考えられる。2型糖尿病においてHbA1c値は、地震後1-2ヶ月では地震前と比較して0.11%低下 (7.33%→7.22%)したが、3-4ヶ月後、6-7ヶ月後、12-13ヶ月後は上昇した。その他の糖尿病において、血糖変動は認められなかった。糖尿病全例を評価した場合、80.6%を占める2型糖尿病の動きと同等であった。2型糖尿病においては、地震後1-2ヶ月でHbA1cが低下した理由として、「ライフラインの早期復旧」と「睡眠量の充足」が、3-4ヶ月後で悪化した要因として、「糖尿病治療薬の不足」と「食事量の不足」が、12-13ヶ月後では、「自宅大規模半壊」と「職場環境の変化」が挙がった。IES-R値と血糖変動の間には有意な相関は認められなかった。

A total of 557 patients were enrolled and data were collected from 13 months before to 13 months after the earthquakes. In patients with type 1 diabetes (T1D) or specific types of diabetes
due to other causes, glycemic control was not altered during the observational period. This glycemic stability in T1D may result from self-management of insulin doses (increase in 7% and decrease in 20% of patients). In patients with type 2 diabetes (T2D), HbA1c was decreased by 0.11% (from 7.33% to 7.22%) at 1-2 months after the earthquakes compared to that before the earthquakes, and increased thereafter.
