当院で平成8年4月1日から平成16年7月31日までに行われた頚椎手術症例対象として調査する。再手術を行った目的として、および神経学的所見をもとにしたスコア（日整会頸髄症治療成績判定基準スコア）痛みのVisual analog scale、急性期の合併症、経年変化に伴う合併症、画像評価としてXp、CT、MRIなどを調査する。現在までに圧迫性脊髄症に対して行われた手術の10年を超えた手術成績を記した報告は非常に少なく、また東京医科歯科大学整形外科で施行している骨化浮上を主体とする前方除圧固定術の長期成績は今のところ報告がなく新たな知見の議論に資する。

Anterior decompression with fusion (ADF) and posterior decompression with laminoplasty (LAMP) are both effective surgical procedures for cervical spondylotic myelopathy (CSM). Previously we reported that ADF provided superior neurologic outcomes to LAMP at 5 years after operation. In this study, we update the data to assess long-term clinical and radiologic outcomes.

主要アウトカム評価項目/Primary outcomes

具体的に術前の患者背景とし、手術時年齢、性別、罹患期間、脊髄モニターリングによって同定された罹患高位、画像所見をパラメーターとし、四肢の神経学的所見を日整会（JOA）スコアで評価を行う。これらの神経学的所見の改善を成績とし、ほかの因子と関連しているかどうかを調査する

Clinical outcomes were evaluated by comparing the recovery rate of the Japanese Orthopaedic Association (JOA) score between the two groups. Radiographs were taken before surgery and every year after surgery. Sagittal alignment of the C2-7 lordotic angle and range of motion (ROM) in flexion and extension were measured before and after surgery.
