本研究は、慢性痛患者では健常者と比較し、Default Mode Networkを中心とした脳のネットワーク異常が生じるという仮説をもとに行う探索的試験である。本研究では、Default Mode Network内外の特定の脳領域間の機能的結合性の強さを、resting state fMRIから得られた画像情報から解析ソフトを用い定量化し、慢性痛患者と健常者で比較する。さらに、慢性痛患者での痛みの強さや心理的要因の指標と特定の脳領域間の機能的結合性との相関についても検討する。
慢性痛患者の中には、痛みの原因となる身体の器質的な障害がないにもかかわらず、強い痛みが遷延化することが多い。resting state fMRIによって慢性痛関連の脳領域を特定し、それらの領域の機能的結合性の異常を明らかにすることで、今後は、痛みの原因の同定に際し、非侵襲的な画像診断ツールとして活用できる可能性がある。将来的には、脳のネットワーク異常を元に戻す手法（ニューロフィードバック療法）を患者に促し、難治性慢性痛の新たな治療法を確立することを目標にしている。また、心理的要因と特定の脳領域間の機能的結合性との関連を明らかにすることにより、慢性痛患者に対する心理的介入が脳内の機能的結合性の改善に寄与するかどうかの評価ツールとしてresting state fMRIを活用することを目標とする。

It has been reported that the altered functional connectivity between brain regions may be associated in the development, prolongation, and augmentation of chronic pain. The objective of this prospective study is to detect the changes in functional connectivity of the brain regions associated with Default Mode Network (DMN) in chronic pain patients by using resting state fMRI. Further, we will examine the associations among the changes in functional connectivity, the intensity of chronic pain, and psychological indicators.

選択基準/Key inclusion criteria

慢性痛患者群：慢性痛がある20歳から75歳の右利き患者を対象とする。慢性痛の定義は3カ月以上持続している痛みとする。30例集める。
健常者群：年齢と性別をマッチさせた1か月以上、痛みに関して治療を受けていない健常者を20例集める。年齢は、20-24歳、25-29歳、30-34歳、35-39歳、40-44歳、45-49歳、50-54歳、55-59歳、60-64歳、65-69歳、70-75歳の範囲内でマッチさせる。

Chronic pain patients group: 20-75 years old, right handed, and patients whose pain has been persisting for more than 3 month.
Healthy control group: Healthy volunteer who were age-matched
