目的1/Narrative objectives1

日本語
分離肺換気では、非換気側の血流が低下し両側換気再開後再灌流性障害に関連するサイトカインが上昇することが知られている。肺の障害は分離肺換気の手術後の最も頻度の高い合併症であり、肺合併症を防ぐたような麻酔管理が必要である。一方、デクスメデトミジンは、抗炎症、抗酸化作用が報告されており、人工心肺や肝臓のプリングル手技に伴う再灌流障害と、炎症性サイトカインの上昇をおさえられることがこれまで報告されている。しかし分離肺換気に伴う再灌流障害に対してデクスメデトミジンが及ぼす影響については知られていない。デクスメデトミジンに分離肺換気による再灌流障害を抑える作用があるとすれば、分離肺換気において、肺合併症の防止に寄与できる。

In isolated lung ventilation, it is known that the non-ventilatory side blood flow decreases and the cytokine associated with reperfusion injury increases after restarting bilateral ventilation. Disorders of the lung are the most frequent complications after the operation of the isolated lung ventilation, and anesthesia management that prevents pulmonary complications is necessary. On the other hand, dexmedetomidine has been reported to have anti-inflammatory and antioxidative effects, and it has been reported that reperfusion injury due to artificial heart-lung and liver prying technique and suppression of elevation of inflammatory cytokines can be suppressed. However, the effect of dexmedetomidine on reperfusion injury associated with isolated lung ventilation is unknown. If dexmedetomidine has an action of suppressing reperfusion injury due to isolated pulmonary ventilation, it can contribute to prevention of pulmonary complications in isolated lung ventilation.

分離肺換気で肺切除術を行い、術後ICUに入る患者

Patients who undergo lung resection with isolated lung ventilation and enter ICU after surgery

除外基準/Key exclusion criteria

日本語
プレセデックスにアレルギーあり、肺および全身性炎症性疾患、BMI 35以上、再手術患者、心機能低下患者（EF40％以下）、Hugh-Jones分類4以上、自己免疫疾患、免疫抑制剤投与中、硬膜外麻酔が適応外の患者
