もの忘れセンターを受診して、認知機能の検査と腸内フローラの解析に同意した、日本語が母国語の①健常者、②MCI患者、③認知症患者。

Patients who visit the memory clinic at the National Center for Geriatrics and Gerontology (NCGG) and agreed to both the medical check-up of cognitive function and the fecal examination to survey gut microbiota.Patients were eligible for the present study if they: (1) can undergo a brain magnetic resonance imaging (MRI); (2) have provided informed consent in writing; (3) are accompanied by a study partner to assess the patient's condition.

除外基準/Key exclusion criteria

日本語
1. 脳腫瘍、硬膜下血腫、後遺症を残した頭部外傷の既往がある場合。
2. 登録時までに実施したCTやMRIで、認知機能に強い影響を与えるような脳梗塞等の局所病変が見つかった場合。
3. 大うつ病や双極性障害、統合失調症の既往、アルコール依存や他の薬物依存の既往、重篤な疾患や状態の安定しない疾患に罹患している場合。
4. ビタミンB1・ビタミンB12・葉酸欠乏、梅毒、甲状腺機能異常があり、治療が困難な場合。
5. CDRやMMSEなどの神経心理検査の実施が困難な者。
6. その他、研究責任者又は分担研究者が登録に適さないと認定した場合。

Patients if they: (1) are unable to undergo MRI examination, or MRI cannot be evaluated due to body movement; (2) having any local lesion such as cerebral infarction(s) detected by MRI before enrollment that can greatly affect the cognitive function; (3) having a history of major psychological disorder, or alcohol/drug abuse as a current serious or unstable disease; (4) are having less than 6 years of education; (5) are having a history of digestive cancer; (6) are judged by an investigator to be ineligible to participate as a study subject

主な結果/Results

日本語
同意取得した181例のうち、適格と判断された128例を解析した（女性59%、平均年齢74歳、MMSE中央値24点）。認知症患者(34例)は、ラクナ梗塞や脳微小出血を有する割合が高かった。T-RFLP法では、認知症患者においてエンテロタイプＩ（バクテロイデス＞30%）の割合が低く（14.7% vs. 44.7%）、エンテロタイプIII（その他の菌種）の割合が高かった（85.3% vs. 50.0%）。多変量解析では、エンテロタイプＩ（オッズ比0.1、95%信頼区間0.02－0.4、P＜0.001）、エンテロタイプIII（オッズ比12.7、95%信頼区間3.3－65.8、P＜0.001）が認知症と強く関連した。

The T-RFLP analysis revealed differences in the composition of the gut microbiome: the number of Bacteroides (enterotype I) was lower and the number of other bacteria (enterotype III) was higher in demented than non-demented patients. Multivariable analyses showed that the populations of enterotype I and enterotype III bacteria were strongly associated with dementia, independent of the traditional dementia biomarkers.
