薬害被害によるHIV感染症に罹患している中高年血友病症例の運動器障害について把握することを目的とする。血友病患者会の際に運動器検診を行い、運動機能の横断的把握をする。またこれを三年間継続することでそれらの縦断的把握もする。

The aim of this study is to survey the musculoskeletal disorders of HIV-infected the middle-aged and elderly hemophilia. It performs a musculoskeletal screening at the time of hemophilia patient advocacy, and the cross-cutting assessment of motor function. By continuing this survey three years, to survey the longitudinal assessment of the motor function.

運動器検診会に参加を希望し、その結果の研究利用に同意し、関節可動域テスト、徒手筋力テスト、握力、10ｍ歩行、ADLに関する質問による調査の実施が可能である症例（ただし10ｍ歩行は必須ではない）。

A case hoping for participation in the fitness test and agree to the study use of the result of range of motion, manual muscle test, a grip strength, the 10m walking test, and ADL questions and investigations(but the 10m walk test is not required.)

除外基準/Key exclusion criteria

評価当日に、治癒していない出血性関節症、関節内出血および筋肉内出血、治癒していない骨折を有している症例。

A case with not healing hemorrhagic arthropathy.
A case with joint bleeds.
A case with intramuscular bleeding.

主な結果/Results

日本語
筋力低下は40歳代から認められた。筋力低下が最も顕著だったのは股関節外転の筋力で、次に肘関節伸展の筋力低下が強かった。歩行速度は、年代が高くなるにつれ、速度および歩幅が低下していた。多くの症例が歩行速度が改善している。これは、運動器検診会の参加自体が、運動機能維持のモチベーションにつながり改善をもたらした可能性を示している。関節可動域は肘関節の伸展が最も制限があり、次に足関節の背屈制限が顕著であった。年代別の関節可動域は、膝関節屈曲および肘関節の屈曲において経年変化が顕著であった。

Muscle weakness has been observed since the 40s. The most significant decrease in muscle strength was hip abduction muscle strength, followed by conspicuous decline in elbow extension strength. The walking speed of the walking speed decreased as the age increased. The range of motion of the joint was most restricted by the extension of the elbow joint, followed by the restriction of the dorsiflexion of the ankle joint. The range of motion was prominent over time in knee flexion and elbow flexion

研究デザインはコホート研究。対象者の募集方法は、はばたき福祉事業団が毎年行っている、運動器検診を兼ねた患者会において、そのデータを研究利用することに同意した症例。調査は三年間継続する。

recruitment:Hemophilia Patients' association "Habataki" holds a Gathering of patients every year. At that time, they perform a kind of Measurement event of motor function by our cooperation. In the participants, those who agree to use their data are the subjects of our study. Our study is continued for three years.
