目的1/Narrative objectives1

日本語
ナビゲーションの使用により正確なコンポーネント設置、適正な下肢アライメントが得られるなど、ナビゲーションの有用性についての報告は多い。しかし、下肢bowingを有するTKA施行患者にナビゲーションの有用性を評価した報告は限られている。下肢bowingの頻度、bowingのTKA冠状断・矢状面アライメント・コンポーネント設置に及ぼす影響、ナビゲーションの有用性について評価することが本研究の目的である。

両側同時TKAを施行する患者を対象とし、片側にナビゲーションを使用し、反対側にはナビゲーションを使用せず、従来法で手術を行う。

変形性膝関節症、膝骨壊死などの膝関節疾患で、当院にてTKAを施行する男女100症例を、年齢を問わず対象とする。

主な結果/Results

日本語
術前の下肢アライメントはナビ群、非ナビ群の2群間で差を認めなかった(表1)。術後は2群間で、膝外側角, α角、大腿骨機能軸に対するコンポーネント設置角に有意差を認めた。大腿骨bowingはナビ群3.5 ± 2.9°、非ナビ群3.3 ± 3.5°、脛骨bowingはナビ群0.6 ±2.0°、非ナビ群0.1±1.8°であった。
大腿骨、脛骨2°以上のbowingはそれぞれ69%、27%、大腿骨3°以上のbowingは52%であり（図3）、過去の報告12),15)と同等またはそれ以上であった。Bowingはアジア人に一般的であることを支持する結果であった。

Femoral bowing varied from -7.4 to 10.9 with an average of 3.0. Tibial bowing varied from -4.1 to 4.6 with an average of 0.4. The femoral component was placed more properly with less outlier in the navigation group. The outlier of the femoral component angle was lower in the non-bowing group than in the bowing group.

両側同時TKAを施行した31症例(男性3例、女性28例) 62膝を対象とした。全例Kellgren-Lawrence grade IVの変形性膝関節症であった。

評価項目/Outcome measures

術前・術後立位下肢長尺正面X線を用いて、以下の項目について計測を行った。
・大腿骨bowing角: 小転子直下から顆部上部までの範囲を4等分し、最上部と最下部のゾーンに引いた中線のなす角度。
外弯(+)、内弯(-)とし、±3°以上を大腿骨bowing有りとした。
・脛骨bowing角: 脛骨を3等分し、上下のゾーンに引いた中線のなす角度12)。外弯(+)、内弯(-)とし、±2°以上を脛骨bowingとした。
・術前・術後hip-knee-ankle angle (HKA)
・術前・術後膝外側角(図2C,F)
・大腿骨機能軸と解剖軸のなす角度: 大腿骨遠位骨切りの参照角度とした。
・大腿骨、脛骨解剖軸に対するコンポーネント設置角度 (α角, β角)
・ 大腿骨機能軸に対するコンポーネント設置角度:　目標角度を90°とし、3°以上の誤差をoutlierとした。

Preoperative coronal bowing were measured, and the subjects were divided into 2 subgroups, i.e. the bowing group and the non-bowing group. Lateral bowing was expressed as plus (+) and medial bowing was expressed as minus (-). Various radiographic parameters, including coronal bowing, lower leg alignment, component position, and outliers were compared between the navigation group and the conventional group.
