選択基準/Key inclusion criteria

1)　原発巣が組織診、細胞診、または画像検査から膵癌であることが確認されている症例で、手術時に剥離断端に病理学的癌遺残の可能性がある(R1)局所進行膵癌例（ただし、原則として経過観察終了時までに、可能な限り組織診あるいは細胞診を実施し、膵管癌であることを確認する）で、MDCTによる進展度診断を行い、上記対象に合致する症例
2)　測定可能病変を有する症例
3)　初回治療症例
4)　下記のUnresectable膵癌の定義を満たす症例
　NCCNガイドライン（Version 2.2016）におけるUnresectableの定義（①-⑤）に該当する症例：①遠隔転移がない（領域外リンパ節転移を含む）②膵頭部腫瘍浸潤により上腸間膜静脈（SMV）および門脈（PV）内腔が閉塞している、あるいは最近位空腸静脈に接触している。膵体尾部腫瘍によるSMV/PV浸潤または閉塞によりSMV/PVが再建不能である。③膵頭部腫瘍が上腸間膜動脈または腹腔動脈に180度以上で接触している。膵頭部腫瘍が第1空腸動脈に接触している。膵体尾部腫瘍が上腸間膜動脈または腹腔動脈に180度以上で接触している。膵体尾部腫瘍が腹腔動脈と大動脈に接触している。
5)　胆道メタリックステント留置症例ではステント素材はニチノール（英: nitinol、ニッケル・チタン合金）製のものとする（胆道ドレナージ症例のみ）。

1. Histopathologically diagnosed as pancreatic ductal adenocarcinoma or adenosquamous carcinoma by cytology, histological examination, or imaging study, and diagnosed as unresectable resectable pancreatic carcinoma (URPC) by MD-CT .
2.Measurable lesion is present.
3.Patients who undergo initial therapy.
4.Meet the definition of URPC in detail: Fulfill the definition of BRPC in NCCN guideline Version 2.
5.Metallic stent made of Nitinol is allowed to use for biliary drainage.

除外基準/Key exclusion criteria

1) 重篤な薬物過敏症、薬物アレルギーの既往のある症例
2) 悪性腫瘍の既往がある症例（※5年以上の無再発期間がある場合、および内視鏡的に治癒切除された粘膜内癌や治癒切除された子宮頸部癌、皮膚の基底細胞癌または扁平上皮がんは登録可）
3) 活動性の感染症を有する症例
4) MRI検査禁忌となる体内金属等を有する症例
5) 閉所恐怖症を有する症例
6) 息こらえ、などの協力が得られない症例
7) コントロール不能な腹水、胸水を有する症例
8) コントロール不能な糖尿病を有する症例
9) コントロール不能なうっ血性心不全、狭心症、高血圧、不整脈を有する症例
10) 神経学的または精神的に重大な疾患の既往、合併がある症例

2. With history of malignant disease within 5 years.
3. Active stutus of infectious disease.
4. With metal in body which is contraindication for MRI.
5. With claustrophobia.
6. Incapability to collaborate in respiration.
7. The presence of uncontrollable ascites.
8. The presence of uncontrollable DM
9. The presence of uncontrollable CHD, angina, HTN, and arrhythmia.
10. The presence of severe neurological, psychological disease or their history.

主な結果/Results

治療前/治療後のADC値は、全パラメータの中で腫瘍細胞消失率と強い相関を認めた（R = 0.630 / 0.714、P <0.001 / <0.0001）。治療後のgrade<IIbと≧gradeIIbとを区別するための治療前後の全腫瘍平均ADC値カットオフ値は1.19/ 1.39×10-3 mm2/sで、R0切除に関して、感度63/88%、特異度70/70%、精度68/75%で予測可能であった。

Pre/post-treatment whole tumor ADC value correlated with tumor cell destruction rate among all parameters (R=0.630/0.714, P<0.001/<0.0001). The post-treatment cutoff value of vascular abut site ADC for discriminating between grade<IIb and >=grade IIb was determined as 1.42x10-3 mm2/s and predicts R0 curability. For histological response, the post-treatment whole tumor ADC cutoff value for discriminating between grade<IIb and >= grade IIb was determined as 1.40x10-3 mm2/s.

術前治療は全例ゲムシタビン・ナブパクリタキセル併用療法を施行し、術前後にMD-CT、拡散強調磁気共鳴画像法（DW-MRI）、PET-CTを施行した。臨床・画像所見の情報のない2人の病理専門医が診断した腫瘍細胞消失率（Evans grade）と以下のパラメーターとの相関関係について独立した2人の生物統計家が分析し、相関関係の強いパラメーターのカットオフ値とその意義を解析した。パラメーター：MD-CTの腫瘍最大径、MRI拡散強調画像の全腫瘍または血管接触部位の見かけの拡散係数（ADC）の平均値、18F-FDG-PETのSUVmax値、腫瘍マーカー19-9値、およびそれらの術前治療前後値、変化率または差。強力なパラメータは、治療的組織学的反応およびR0の予測能力について評価した。

We elucidate correlation between pre/post-treatment whole tumor apparent diffusion coefficient (ADC) value and tumor cell destruction rate. We verify whether post-treatment vascular abut site ADC value predicts R0 curability of borderline resectable pancreatic cancer (BRPC) and locally advanced pancreatic cancer (LAPC).　Analyses for correlation between percent tumor cell destruction and various parameters were performed. Strong parameters were assessed for ability to predict therapeutic histological response and R0 curability.
